Itoffers an opportunity for reducing the treatment duration considerably and harbours thepotential for less exposure of healthy tissue to higher radiation doses. However, evidenceissued from randomized trials is limited. European and American experts call our attentionto the potential dangers of widespread implementation of these techniques without anylong-term data on outcome and ask for complete information of patients on the potentialhazards and risks if accelerated partial breast irradiation is used instead of whole breastirradiation.(Belg J Med Oncol 2011;5:3-7)

Dans le contexte d'une augmentation progressiveet continue de la charge de travail et afin de réduireles contraintes humaines et techniques, une radiothérapie pluscourte est aujourd'hui utilisée. Ces schémas hypo-fractionnéssont validés par des essais randomisés et publiés. Sur la basede ces publications, un nouvel arbre décisionnel a été établi quipermet d'obtenir des résultats équivalents en contr©ole local touten gardant un taux de complications comparable, voire m©emeinférieur au schéma classique.ENGLISH: SUMMARY : Postoperative adjuvant radiotherapy for breastcancer is usually applied in twenty five fractions of 2 Gy onthe whole breast followed by a boost dose on the initial tumoursite. Facing a continuous rise of the workload in radiotherapydepartments, investigators have tried to reduce the burdenof such a treatment approach both for the patients and thedepartments as available resources are not unlimited. Thesehypo-fractionated treatment schedules have been tested andvalidated in randomized controlled trials. Based on this evidencea new treatment algorithm has been built up as it hasbeen shown that those schedules do provide similar levels oflocal control with comparable or an even lower risk of toxicity.
