Megjegyzések:The Debrecen Stroke Unit covers a catchment area of 210,000 inhabitants in eastern Hungary. The unit was established at the Department of Neurology of the University Hospital in 1974 and has 23 beds, 7 of which have monitoring facilities. The unit treats about 600 patients with acute cerebrovascular diseases annually - about 60% of all hospitalised stroke cases in the region. Overall, 18 registered nurses and 4 nurse helpers work for the unit. Computer tomography is performed in over 90% of cases. Carotid duplex ultrasound and echocardiography are part of the routine examinations in ischaemic strokes. Delay from onset of stroke to hospital arrival is the main barrier against the use of rt-PA. Average length of stay is 12 days; a lack of rehabilitation and nursing capacities sometimes delays discharge of dependent patients. The hospital is reimbursed the costs of stroke care based on DRG.

Megjegyzések:The central nervous system is clinically affected in about half of the systemic lupus erythematosus (SLE) patients. Mean blood flow velocities in the middle cerebral artery measured by transcranial Doppler ultrasound (TCD), before acetazolamide administration, were significantly higher in 9 patients without neurological symptoms and without MRI abnormalities (74.5 ±3.1 cm/s; mean ± SEM) than in 10 healthy controls (56 ± 2.8 cm/s) or in hemispheres of 6 SLE patients with neurological symptoms (44.5 ±2.6 cm/s). After acetazolamide, the response was significantly higher in symptom-free SLE patients (125 ± 4.6 cm/s) and lower in 6 patients with focal neurological symptoms in the affected hemisphere (66.5 ± 3.1 cm/s) compared with healthy controls (87.3 ± 4.1 cm/s). Our results suggest that the severity of impairment of the cerebral vessels is detectable by TCD in SLE patients. The acetazolamide test is suggested to be used in the follow-up of this disease.

Megjegyzések:Background: Autopsy studies show a higher prevalence of circle of Willis anomalies in brains with signs of ischemic infarction. Our goal was to examine the collateral function of the circle of Willis in ischemic stroke patients and to assess in a case-control study if a collateral deficient circle of Willis is a risk factor for ischemic stroke in patients with severe internal carotid artery (ICA) occlusive disease. Methods: Our case-control study included 109 patients with an acute ischemic stroke in the anterior circulation and 113 patients with peripheral arterial disease and no known history of cerebral ischemia. The collateral function of the anterior and posterior communicating arteries of the circle of Willis was assessed by means of transcranial color-coded duplex ultrasonography (TCCD) and carotid compression tests. Results: TCCD was successfully performed in 75 case patients (mean age 64 years, range 41-91 years) and in 100 control patients (mean age 61 years, range 35-89 years). In 26 cases and 19 controls, a ? 70% stenosis or occlusion of the ICA was found. A nonfunctional anterior collateral pathway in the circle of Willis was found in 33% of the cases and in 6% of the controls (p < 0.001). The posterior collateral pathway was nonfunctional in 57% of the cases and in 43% of the controls (p = 0.02). In patients with severe ICA occlusive disease, the odds ratios of a nonfunctional anterior and a nonfunctional posterior collateral pathway were 7.33 (95% confidence interval, CI, = 1.19-76.52) and 3.00 (95% CI = 0.77-12.04), respectively. Conclusions: Patients who suffer ischemic stroke in the anterior circulation have a higher incidence of collateral deficient circles of Willis than those with atherosclerotic vascular disease without ischemic cerebrovascular disease. The presence of a nonfunctional anterior collateral pathway in the circle of Willis in patients with severe ICA occlusive disease is strongly associated with ischemic stroke.

Megjegyzések:The major limitation of native transcranial colour-coded duplex sonography (TCCS) in older stroke patients is the relatively frequent occurrence of an insufficient temporal window. Our goal was to investigate the relationship between the in vivo Doppler ultrasound image quality of the temporal bone, and computed tomography (CT)-determined thickness, density, and homogeneity of cadaver temporal bone. METHODS: Thirty-three moribund neurological patients who eventually died were examined by TCCS using the transtemporal approach. The sonographer categorized the quality of the TCCS image (excellent, intermediate, and poor). During autopsy, a rectangular sample of the temporal squama was removed, which corresponded to the area of the in vivo acoustic window. The thickness of the whole temporal bone, cortical, and cancellous (= diploe) bone as well as the density and homogeneity were determined by high-resolution CT. RESULTS: Thirty-seven temporal bones were obtained. The quality of the acoustic window was classified as excellent in 13, intermediate in 6 and poor in 18 cases. A significant correlation between the complete bone thickness, as well as between the absolute thickness of the diploe and the quality of the acoustic window was found: the thinner the bone/diploe, the better the colour Doppler signal. The thickness of the cortical plates and the homogeneity of the bones were identical in the three image quality categories. CONCLUSION: The transtemporal TCCS image quality depends mainly on the thickness of the cancellous component of the temporal bone.

Megjegyzések:The aim of the present work was to investigate intima media thickness (IMT) in healthy and in hypertensive adolescents and its influencing factors. METHODS: 103 hypertensive and 58 healthy adolescents were studied. IMT was measured in the common carotid artery using B-mode ultrasonography. Additionally, laboratory parameters (blood glucose, cholesterol, triglycerides, HDL- and LDL-cholesterols) and left ventricular mass indices were obtained. RESULTS: IMT in the common carotid artery was higher in hypertensive adolescents (means +/- SD: 0.55 +/- 0.11 mm) than in healthy control subjects (0.48 +/- 0.08 mm, p < 0.001). Similarly, a higher left ventricular mass index was measured in hypertensive (103.2 +/- 30.6 g/m(2)) than in healthy teenagers (91.1 +/- 25.2 g/m(2), p < 0.001). In general, IMT in adolescents was associated with age, weight, body mass index, left ventricular mass index and average systolic and diastolic blood pressure values of the subjects. By assessing the multivariate association between IMT and other factors, intima-media thickness was only associated with age and left ventricular mass index of the hypertensive subjects and was independent from all the investigated factors in normotensive controls. CONCLUSIONS: Our data suggest an ongoing target-organ damage in adolescent hypertension. These patients need to be subjected to early diagnostic methods, treatment and a regular follow-up, in order to avoid severe clinical manifestations of secondary target-organ damage due to hypertension.
