Megjegyzések:Objective: Riboflavin UVA (collagen crosslinking [CXL]) treatment for keratoconus is a method for stabilizing or improving corneal properties. Our aim was to evaluate changes in corneal topography indices after CXL. Methods: A total of 38 eyes of 25 patients (mean age 29.36 ± 9.7 years) were treated with CXL technique. The follow-up period was 36 months. Slit-lamp examination, visual acuity tests, and pachymetry measurements were performed; in order to obtain the following numerical data, corneal topography (TMS-4; Tomey) was used: simulated keratometry (K1, K2), CYL (cylinder value), SAI (surface asymmetry index), IAI (irregular astigmatism index), SRI (surface regulatory index), ACP (average corneal power), and CEI (corneal eccentricity index). Treatments were performed with In-Pro CCL-Lix (Germany) equipment. Results: The values of mean K1 (p = 0.82), cylinder (p = 0.73), SAI (p = 0.99), IAI (p = 0.82), SRI (p = 0.73), ACP (p = 0.73), K2 (p = 0.8), and CEI (p = 0.99) showed no significant changes at the end of the examination period. Uncorrected visual acuity remained unchanged (p = 0.32). However, best-corrected visual acuity changed from 0.21 ± 0.17 to 0.12 ± 0.11 in logMAR scale, but this deviation was not considered significant (p = 0.08). No change was observed in intraocular pressure (p = 0.79). Cornea remained clear and during follow-up period no corneal thinning was observed (p = 0.78). Conclusion: Our results suggest that corneal collagen CXL can be a successful treatment at certain stages of keratoconus, offering a way to stop progression in keratometric indices.

Megjegyzések:Pellucid marginal degeneration (PMD) is an uncommon cause of inferior peripheral corneal thinning disorder, characterized by irregular astigmatism. We analyzed a case of bilateral PMD patient and treated one eye with corneal collagen cross-linking (CXL) therapy. Corneal topography was characteristic for PMD. Visual acuity, slitlamp examinations, tonometry, and corneal thickness were observed. Simulated keratometric and topographic index values were detected with corneal topography. Uncorrected, LogMAR visual acuity has improved from +0.8 to +0.55 during the 6 months and +0.3 during the 8 months follow-up after CXL. Pachymetry values and intraocular pressure showed no changes. Keratometric values and topografic indexes disclosed no progression of the disease. CXL may postpone or eliminate the need of corneal transplantation in cases with PMD.

Megjegyzések:PURPOSE: The study was conducted to assess the central corneal thickness (CCT) of the healthy cornea with a recently developed noncontact specular microscope (EM-3000; Tomey) and compare the results with those measured with a contact specular microscope and an ultrasound pachymeter. Agreement between measurements taken by 2 investigators was also studied. METHODS: The right eyes of 41 healthy individuals who had negative history of contact lens wear, ophthalmic disease, or ocular surgery were examined. The CCT was determined sequentially with a noncontact specular microscope, a contact specular microscope (EM-1000; Tomey), and an ultrasound pachymeter (AL-2000; Tomey). Each evaluation with the specular microscopes was performed by 2 independent operators. RESULTS: A significant difference was detected in pachymetry measurements among the 3 instruments (p=0.01; analysis of variance). The mean CCT values were lower measured with the ultrasound pachymeter (537+/-30 microm) than the contact endothelial microscope (543+/-37 microm, p=0.17, Student t-test) and the noncontact microscope (549+/-33 microm, p<0.0001) (operator 1). There was no statistically significant difference in CCT measurements between the 2 endothelial microscopes (p=0.19). We found significant correlations (p<0.0001) in thickness measurements between each pair of instruments (r=0.91, noncontact microscopy and ultrasound pachymetry; r=0.74, noncontact and contact microscopy; r=0.72, contact microscopy and ultrasound pachymetry; Spearman rank correlation). CONCLUSIONS: The strong correlations among the 3 pachymetry devices suggest that the tested instruments provide reliable measurements; however, they cannot be used interchangeably.

Megjegyzések:To assess astigmatism induced by paired opposite clear corneal incision (OCCI) compared to single incisions and to investigate factors on which its effect depends.The study comprised 326 eyes from 326 patients. In patients with &lt;1.0 D corneal astigmatism, clear corneal incision was performed (CC group, n = 245), and in those with &gt;1.0 D preoperative corneal astigmatism, a second, opposite clear corneal incision was made (OCCI group, n = 81). Both groups consisted of superior, temporal, and oblique subgroups according to the steepest meridian. Prior to the surgery, and after surgery, repeated keratometry was performed and surgically induced astigmatism (SIA) was calculated using vector analysis.Two weeks after the surgery, the SIA was 0.67 ± 0.44 D in the CC group and 1.15 ± 0.65 D in the OCCI group; 9 weeks after the operation, it was 0.61 ± 0.43 D in the CC group and 0.99 ± 0.57 D in the OCCI group (p&lt;0.001). The SIA showed no significant postoperative change in our groups. Opposite clear corneal incision reliably induces about 1.0 D of corneal astigmatism on the steep meridian in proportion to the rate of preoperative astigmatism. Its magnitude is independent of the location of the incisions and its effect remains unchanged during the postoperative period

Megjegyzések:Purpose. Ophthalmic parameters can be measured with different techniques and devices. Our aim was to measure anterior segment parameters using 2 optical devices and to compare the data to ultrasonic technique. Methods. Axial length (AL) was measured with IOLMaster; anterior chamber depth (ACD) was determined using IOLMaster, Visante® optical coherence tomography (OCT), and Pentacam® HR; and central corneal thickness (CCT) were measured with Pentacam® HR and with ultrasound. Results. In 45 eyes of 45 patients (age 62.73±15.31), the AL was 23.35±1.49 mm. The CCT was 541.7±39.9 gamma m with Visante OCT, 549.7±39.2 gamma m with Pentacam® HR, and 534.3±41.3 gamma m with ultrasonic device. The ACD was 3.0±0.42 mm with IOLMaster, 3.26±0.95 mm with Visante OCT, and 3.06±0.41 mm with Pentacam® HR. In case of CCT measurements, data obtained with Pentacam HR correlated with the results of ultrasonic measurements (r=0.89; p<0.001) and Visante OCT measurements (r=0.88; p<0.001). Regarding ACD results, a significant correlation was observed between Pentacam HR and IOLMaster (r=0.95; p<0.0001) but they did not significantly correlate to Visante OCT data (r=0.23; p=0.12). Correlation was significant between AL and ACD measured with IOLMaster (r=0.67; p<0.001). Conclusions. The CCT and ACD data show strong correlation between different devices.

Megjegyzések:PURPOSE: To evaluate the effect of plaque radiation therapy using beta-radioactive isotope for anterior segment tumors on the density and morphology of endothelial cells. ? METHODS: Endothelial cell density (ECD) and morphometry in 15 eyes of 15 patients with ciliary body tumor was examined prior to and 6 months after radiation therapy. After irradiation, central ECD values were also compared with peripheral (2.0 mm from limbus) ECD values measured around the plaque. The ECD, average cell area, coefficient of variation of cell area, and pachymetry measurements were conducted with contact specular microscopy.? RESULTS: The mean corrected ECD values prior to irradiation were 2147 ? 128 cells/mm2 and 2050 ? 108 cells/mm2 after the radiation therapy. After irradiation, the mean peripheral ECD values were 2056 ? 101 cells/mm2. A significant decrease in ECD values was observed after radiation (p = 0.007). Peripheral ECD values measured around the plaque showed no significant difference (p = 0.86) as compared to central ECD values.? CONCLUSIONS: According to our measurements, plaque therapy for tumors in the anterior segment decreases ECD significantly, but not highly, even in case of plaques containing beta-radiation isotope, and the plaques are not placed direct on the cornea surface. The decreased ECD causes no changes in corneal thickness or transparency, but it may have an influence on a subsequent cataract surgery, which generates further endothelial loss.
