Megjegyzések:The release of different cytokines and mediators in tears of patients with keratoconus (KC) wearing contact lenses (CLs) may contribute to the pathology of KC. METHODS: Cohort study was established in patients with KC wearing rigid gas permeable (RGP) CL (group I), patients with ametropia wearing silicone hydrogel (Si-Hi) CL (group II) and ametropic patients wearing RGP CL (group III). RESULTS: Our findings indicate that before CL wear, the release of epidermal growth factor (EGF) and tissue-type plasminogen activator (t-PA) was attenuated, whereas matrix metalloproteinase (MMP)-9, interleukin (IL)-6, chemokine (C-C motif) ligand 5 (CCL5), IL-13, and plasminogen activator inhibitor (PAI)-1 were enhanced in KC compared with ametropes. An increasing linear trend over time was found for MMP-9, EGF, and CXCL8 in KC and MMP-9, MMP-13, IL-6, and CXCL8 in group III. Significant differences were observed in the linear trend over time between groups I and III for MMP-13 and tissue inhibitor of metalloproteinases (TIMP)-1; between groups I and II for MMP-9 and CXCL8; and between groups III and II for MMP-9, CXCL8, and MMP-13. In KC, the release of MMP-9 at week 6 and nerve growth factor (NGF) at 10 min was higher, but NGF at week 2 was lower than that in group II. The release of MMP-13 and NGF at week 2 and 6 were lower in the KC group as compared with group III, and similarly, with IL-6 and CXCL8 at week 2 and PAI at all time points. CONCLUSIONS: Contact lens wear can influence the levels and dynamics of various mediators in the tears of patients with KC that might have an impact on the progression of the disease.

Megjegyzések:A 25-year-old woman who developed transient neurological abnormalities after scuba diving is reported. The subsequent day she experienced transient left-side monocular blindness. Arterial ocular occlusion in apparently healthy young women is unusual, and a search for the cause of this devastating vascular event is mandatory. Occlusion of the left branch retinal artery, total occlusion of the left internal carotid artery, and a petrous apex epidermoid were found, together with a shortened prothrombin time (INR: 0.73), a slightly elevated serum cholesterol level (6.1 mmol/l) and combined thrombophilia (elevated FVIIIC plus type 2 sticky platelet syndrome). This case underlines the complex mechanism of thromboembolic diseases, and the importance of the acquired trigger (in the present case scuba diving) in addition to the long-term anatomical and biochemical risk factors.

Megjegyzések:To report a detailed spectroscopic analysis of explanted hydrophilic acrylic intraocular lenses (IOLs) that were removed because of postoperative opacification of the lens optic. Methods: Thirteen Hydroview H60M (Bausch & Lomb Surgical) IOLs were explanted from 13 different patients on average 56 months after phacoemulsification and IOL implantation. All patients had decreased visual acuity because of a fine granularity of the optical surface of the IOLs. The surface was investigated by gross, microscopic, histochemical and scanning electron microscopic analysis, and the elemental composition of the opacified IOLs was determined by X-ray fluorescence spectroscopy (XRF). The spectrograms were compared to three different originally packed and never-implanted hydrophilic acrylic IOLs. Results: Light and scanning electron microscopy of the optical surface of explanted IOLs revealed multiple fine granular deposits varying in size and shape that were positive for alizarin red. XRF confirmed that the explanted IOLs contained not only the previously reported calcium and phosphorous (calcium apatite), chlorine, silicone, sodium, aluminum and magnesium but also iron, sulfur, potassium as well as lesser amounts of iodine, zinc, strontium and yttrium. Conclusion: This is the first spectroscopic analysis determining the content of more than 10 elements of explanted and originally packed never-implanted hydrophilic acrylic IOLs. The possible origin of the different elements obtained from the spectrograms and their implications are discussed.

Megjegyzések:Pro- (Interleukin-6 (IL-6)) és anti-inflammatorikus (Interleukin-10 (IL-10)) citokinek meghatározása perforáló keratoplasticán átesett betegek könnyéből (különböző posztoperativ időszakban). Összehasonlítani ezen citokinek szintjét a komplikáció nélküli és a kilökődési reakción átesett betegek esetében. Betegek és módszerek: Prospektív módon, 12 szaruhártya-átültetésen átesett betegtől stimulálás nélkül könnyet gyűjtöttünk a posztoperativ 14 hónap során. A vizsgált időszakban 3 betegnél kilökődési reakció zajlott, míg 9 beteg esetében a transzplantátumok komplikáció mentesek voltak. A levett könnyminták IL-6 és IL-10 koncentrációját áramlási citométerrel mértük meg. Eredmények: Az IL-6 koncentrációja szignifikánsan magasabb (p=0,009), míg az IL-10 koncentrációja szignifikánsan alacsonyabb volt (p=0,008) a kilökődési reakción átesett betegeknél, a komplikációmentes esetekkel összehasonlítva. Az IL-6/IL-10 arány a vizsgált időszakban végig szignifikánsan magasabb volt (p=0,0231) a kilökődési reakción áteső betegeknél. Következtetések: A könnyben mért emelkedett IL-6 és az ezzel együtt járó csökkent IL-10 szint a szaruhártya kilökődés indikátora lehet. Ezek a könnyben kimutatott változások azonban a transzplantáció végső kimenetelét nem jelzik előre.

Megjegyzések:This was a kinetic study of inflammatory cytokine levels in postoperative tear samples from penetrating keratoplasty (PKP) patients with or without corneal rejection. In a prospective design, nonstimulated tears were collected from the affected eyes of 12 patients at regular intervals for 12-14 months following PKP. Nine patients retained clear grafts, whereas three suffered endothelial rejection of the corneal graft within 14 months. The concentrations of the cytokines IL-1beta, IL-6, TNF-alpha, IL-8, IL-10, and IL-12p70 were measured via cytometric bead array technology. The postoperative concentrations of the cytokines in the tears varied among the patients, but exhibited similar alteration patterns in each eye tested. The concentrations of IL-6 and IL-8 were significantly higher (P = 0.009 and P = 0.01, respectively), whereas those of IL-10, TNF-alpha, and IL-12p70 were significantly lower (P = 0.008, P = 0.006, and P = 0.0009, respectively) in the tear samples from the patients with corneal rejection as compared with those with uncomplicated corneal grafts. The ratios IL-6/IL-10 and IL-8/IL-10 were significantly higher (P = 0.0231 and P = 0.015, respectively), and TNF-alpha/IL-10 was significantly lower (P = 0.045) throughout the examination period in the patients with endothelial rejection. The enhanced release of IL-6 and IL-8 into the tears of patients with corneal graft rejection concomitant with decreased concentrations of IL-10, TNF-alpha, and IL-12p70 may possibly serve as an indicator of the rejection process. However, due to the large variation in the cytokine concentrations, the observed changes in tear composition do not categorically predict the final graft outcome.

Megjegyzések:To determine the levels of interleukin 6 (IL-6) and interleukin 8 (IL-8/CXCL-8) in tears collected from the eyes of normal individuals and of patients with different irritative eye diseases, in order to acquire information on the immunological changes occurring during the early postoperative period following various forms of eye surgery, including penetrating keratoplasty (PKP). METHODS: IL-6 and IL-8 levels were measured with the aid of human ultrasensitive ELISA kits in the non-stimulated tears of patients in the early postoperative period following PKP or cataract operation, and of patients with acute bacterial conjunctivitis or with a corneal foreign body. The IL-6 and IL-8 concentrations, the total amounts released in a given time and the rates of their release were calculated. RESULTS: A significant increase in IL-6 release was observed in all patient groups compared with the normal controls (p < or = 0.003). The IL-8 release levels were significantly higher in the tears of all patient groups (p < or = 0.03), except for the cataract operation group, where the IL-8 release was not significantly higher (p = 0.053) than in the control samples. No significant differences in IL-6 or IL-8 release were observed when the various patient groups were compared with each other. CONCLUSION: The release of IL-6 and IL-8 into the tears is enhanced in various anterior segment eye diseases, and this may be used as an indicator of various inflammatory reactions in the early postoperative period.
