Megjegyzések:Due to a homozygous Gly204Arg mutation in the factor X(FX) gene no detectable FX antigen was found in the plasma of a one-year old patient with severe bleeding diathesis. The amino acid replacement destabilized the disulfide bond that holds the two FX chains together, decreasing the interaction between the Cys201-Cys206 loop region and the region connecting the EGF2 and serine protease domains. Both Gly204 FX and Arg204 FX were synthesized in transfected cells, but only the wild type protein became secreted. The mutant protein was diverted from the normal secretory pathway and retained at the trans Golgi-late endosome level.

Megjegyzések:Factor XIII (FXIII) activity and antigen levels were determined in 955 patients investigated by coronary angiography. Patients were sub-grouped according to the presence or absence of coronary sclerosis (CS+, CS-) and a positive history of myocardial infarction (MI+, MI-). In females, but not in males, adjusted FXIII activity and antigen levels were significantly elevated in the CS+MI+ group compared to in the CS+MI- group. FXIII levels in the upper tertile were associated with significantly increased risk of MI in females, but not in males.

Megjegyzések:In the platelets of a type II Glanzmann thrombasthenia patient, the amount of glycoprotein (GP) IIb and IIIa was significantly reduced. Three novel mutations were identified in the GPIIb gene (c.440C->G/p.Leu116Val, c.1772_1773insG/p.Asp560 GlyfsX16 and c.2438C->A/p.His782Asn). p.Leu116Val did not represent a causative mutation. The c.1772_1773insG mutation resulted in an early stop codon and nonsense mediated decay of mRNA. When expressed in transfected BHK cells, the truncated protein was unable to form complex with GPIIIa. The p.His782Asn mutation compromised transport of the pro-GPIIb/IIIa complex from the endoplasmic reticulum to the Golgi, hindering its maturation and surface expression.

Megjegyzések:The prothrombotic state resulting from the elevation of certain clotting factors and suppression of fibrinolysis con- tribute to the risk of atherothrombotic diseases. Peripheral artery disease (PAD) has been associated with elevated plasma fibrinogen concentration, increased plasminogen activator inhibitor (PAI-1) level and decreased tissue plas- minogen activator (tPA) activity. 1-3 Coagulation factor XIII (FXIII) circulates in association with fibrinogen and plays a central role in fibrin stabilization and fibrinolysis. 4 We have recently shown that in women with coronary steno- sis and history of myocardial infarction adjusted plasma FXIII levels are elevated as compared to clinical controls, and in women, but not in men, elevated FXIII level repre- sents a 2.5-3.0-fold increased risk of myocardial infarc- tion. 5 In this study we investigated FXIII levels in patients with PAD and determined the risk of PAD conferred to patients by elevated FXIII level.
