Megjegyzések:Acquired factor XIII (FXIII) deficiency due to autoantibody against FXIII is a very rare severe hemorrhagic diathesis. Antibodies directed against the A subunit of FXIII, which interfere with different functions of FXIII, have been described. Here, for the first time, we report an autoantibody against the B subunit of FXIII (FXIII-B) that caused life-threatening bleeding in a patient with systemic lupus erythematosus. FXIII activity, FXIII-A(2)B(2) complex, and individual FXIII subunits were undetectable in the plasma, whereas platelet FXIII activity and antigen were normal. Neither FXIII activation nor its activity was inhibited by the antibody, which bound to structural epitope(s) on both free and complexed FXIII-B. The autoantibody highly accelerated the elimination of FXIII from the circulation. FXIII supplementation combined with immunosuppressive therapy, plasmapheresis, immunoglobulin, and anti-CD20 treatment resulted in the patient's recovery. FXIII levels returned to around 20% at discharge and after gradual increase the levels stabilized above 50%.

Megjegyzések:Congenital factor XIII (FXIII) deficiency is a rare,autosomal-recessive bleeding diathesis that can be associatedwith impaired wound healing and recurrent miscarriages.Delayed umbilical cord bleeding is a typicalearly presentation. Intracranial haemorrhage developsfar more frequently than in any other inherited bleedingdisorder. Therefore, life-long prophylactic factor supplementationis mandatory if FXIII activity is below 1%.Subcutaneous, mucosal, intramuscular or joint bleedingand postoperative haemorrhage can also occur [1].Previously, plasma-derived sources of FXIII wereavailable for preventive and on-demand therapy. A noveloption for FXIII replacement is a recombinant FXIII(rFXIII) concentrate (catridecacog, NovoThirteen , NovoNordisk, Bagsvaerd, Denmark) that contains the A subunit(FXIII-A) from the tetramer of plasma FXIII-A2B2.Recombinant FXIII-A2 (rFXIII-A2) combines with endogenousB subunit (FXIII-B) to form stable FXIII-rA2B2reaching a half-life of 6?12 days [2,3]. Currently rFXIII islicensed for monthly prophylaxis in a dose of 35 IU kg 1in severe congenital FXIII-A subunit deficiency [www.ema.europa.eu, www.fda.gov]. The efficacy and safety ofrFXIII have not been tested in acute bleeding. However,pharmacokinetic profile of rFXIII [3] supports the hypothesisthat it is also suitable for on-demand therapy [1].This letter reports the effective use of rFXIII for thetreatment of a major bleeding in a patient with severecongenital FXIII-A deficiency.

Megjegyzések:Factor XIII is a coagulation factor with multiple plasmatic and cellular functions part of which is outside of the field of traditional hemostasis. The aim of the review is to provide a brief summary on the relationship between coagulation factor XIII (FXIII) and the cells of the immune system. In the first part the structure and biochemical functions of plasma and cellular FXIII are briefly summarized. Then, the interaction between leukocytes and factor XIII is discussed. This part includes the activation of FXIII by human neutrophil elastase, the down-regulation of activated FXIII (FXIIIa) by granulocyte proteases within the clot, and the effect of FXIIIa on leukocytes. In the following part data on the expression and subcellular distribution of FXIII in monocytes/macrophages are summarized. Another part of the review is devoted to changes of FXIII expression during monocyte differentiation and monocyte activation by the classical or the alternative pathway. In the final part reports on the possible functions of cellular FXIII in monocytes and macrophages are evaluated.

Megjegyzések:Our aim was to investigate the association of elevated homocysteine (Hcy) and lipoprotein(a) Lp(a) with the prevalence of coronary artery disease (CAD) and myocardial infarction (MI) and to investigate their interaction in both genders.MATERIALS AND METHODS:955 (male/female: 578/377) consecutive patients admitted for coronary angiography were enrolled in the study. Lp(a), Hcy, vitamin B12, folic acid, MTHFR C677T polymorphism and traditional risk factors were determined.RESULTS:619 patients had significant (?50%) stenosis (CAD+) and 341 had MI (MI+). CAD-MI- cases (n=302) were considered as controls. Adjusted Hcy levels were significantly elevated only in the female CAD+MI+group that was related to decreased vitamin B12 levels. Lp(a) was elevated in the CAD+MI+group of both genders. Folic acid levels and MTHFR T677 allele frequency did not show significant difference. Moderate hyperhomocysteinemia (Hcy >15?mol/L) or elevated Lp(a) (>300mg/L) increased the risk of CAD (OR 2.27, CI 1.36-3.80 and OR 1.64, CI 1.03-2.61, respectively) and MI (OR 2.52, CI 1.36-4.67 and OR 1.89, CI 1.06-3.38, respectively) only in women. Only simultaneous but not isolated elevation of Hcy and Lp(a) conferred a significant, 3.6-fold risk of CAD in females and even higher (11-fold) risk in young females, which suggested an interactive effect.CONCLUSIONS:Moderate hyperhomocysteinemia or elevated Lp(a) level associated with a risk of CAD and MI only in women. While isolated elevation of one of the two parameters represented a mild risk of CAD, their combined elevation highly increased the risk in females. No such effect was observed in males.

Megjegyzések:Val34Leu polymorphism of the A subunit of coagulation factor XIII (FXIII-A) is located in the activation peptide (AP) just 3 amino acids away from the thrombin cleavage site. This mutation has been associated with a protective effect against occlusive arterial diseases and venous thrombosis; however, its biochemical consequences have not been explored. In the current study it was demonstrated that the intracellular stability and the plasma concentration of FXIII of different Val34Leu genotypes are identical, which suggests that there is no difference in the rate of synthesis and externalization of wild-type and mutant FXIII-A. In contrast, the release of AP by thrombin from the Leu34 allele proceeded significantly faster than from its wild-type Val34 counterpart. By molecular modeling larger interaction energy was calculated between the Leu34 variant and the respective domains of thrombin than between the Val34 variant and thrombin. In agreement with these findings, the activation of mutant plasma FXIII by thrombin was faster and required less thrombin than that of the wild-type variant. Full thrombin activation of purified plasma FXIII of different genotypes, however, resulted in identical specific transglutaminase activities. Similarly, the mean specific FXIII activity in the plasma was the same in the groups with wild-type, heterozygous, and homozygous variants. Faster activation of the Leu34 allele hardly could be associated with its presumed protective effect against venous thrombosis. No such protective effect was observed in a large group of patients with familial thrombophilia.

Megjegyzések:Alpha2-plasmin inhibitor (?2-PI) has a heterogeneous composition in the plasma. Both N- and C-terminal cleavages occur that modify the function of the molecule. C-terminal cleavage converts the plasminogen-binding form (PB-?2-PI) to a non-plasminogen-binding form (NPB-?2-PI). N-terminal cleavage by soluble fibroblast activation protein (sFAP) results in a form shortened by 12 amino acids, which is more quickly cross-linked to fibrin. The p.Arg6Trp polymorphism of ?2-PI affects N-terminal cleavage. In this work, we aimed to investigate the association between ?2-PI heterogeneity and the risk of venous thromboembolism. Two hundred and eighteen patients with venous thromboembolism (VTE) and the same number of age and sex-matched healthy controls were enrolled. Total-?2-PI, PB-?2-PI and NPB-?2-PI antigen levels, ?2-PI activity, sFAP antigen levels and p.Arg6Trp polymorphism were investigated. Total-?2-PI and NPB-?2-PI levels were significantly elevated in VTE patients, while PB-?2-PI levels did not change. Elevated NPB-?2-PI levels independently associated with VTE risk (adjusted OR: 9.868; CI: 4.095?23.783). Soluble FAP levels were significantly elevated in the VTE group, however, elevated sFAP levels did not show a significant association with VTE risk. The ?2-PI p.Arg6Trp polymorphism did not influence VTE risk, however, in the case of elevated sFAP levels the carriage of Trp6 allele associated with lower VTE risk. Our results showed that the elevation of total-?2-PI levels in VTE is caused by the elevation of NPB-?2-PI levels. Elevated sFAP level or p.Arg6Trp polymorphism alone did not influence VTE risk. However, an interaction can be detected between the polymorphism and high sFAP levels.

Megjegyzések:The effect of factor XIII A subunit (FXIII-A) Val34Leu polymorphism on the risk of coronary artery disease (CAD) has been extensively studied. In this study we investigated how FXIII-A Val34Leu genotypes influence plasma factor XIII levels in patients with coronary sclerosis (CS) and myocardial infarction (MI) and how fibrinogen level modulates this effect. PATIENTS AND METHODS: 955 consecutive patients admitted for coronary angiography were categorized according to the presence or absence of significant CS and the history of MI. The frequency of FXIII-A Val34Leu polymorphism, fibrinogen, FXIII activity and antigen levels were determined. RESULTS AND CONCLUSIONS: CS or MI decreased FXIII levels in patients homozygous for FXIII-A Leu34 allele, but not in heterozygous or wild type patients. In the subgroup of patients with CS, but without the history of MI no significant effect was detected, which suggests that MI has a more prominent role. The specific activity of plasma FXIII was independent of FXIII-A Val34Leu genotype. FXIII and fibrinogen levels significantly correlated in CS+ and MI+ patients. In MI+ patients of Leu/Val or Leu/Leu genotypes and with fibrinogen levels in the lowest quartile, FXIII levels were lower than in the same patient groups, but with higher fibrinogen level. The low-scale continuous activation of blood coagulation in CAD patients could lead to parallel FXIII and fibrinogen consumption. As the same amount of thrombin activates more Leu34 FXIII than Val34 FXIII, increased FXIII consumption might be responsible for the decreased FXIII levels in Leu34 homozygous CAD patients.

Megjegyzések:Factor XIII (FXIII) activity and antigen levels were determined in 955 patients investigated by coronary angiography. Patients were sub-grouped according to the presence or absence of coronary sclerosis (CS+, CS-) and a positive history of myocardial infarction (MI+, MI-). In females, but not in males, adjusted FXIII activity and antigen levels were significantly elevated in the CS+MI+ group compared to in the CS+MI- group. FXIII levels in the upper tertile were associated with significantly increased risk of MI in females, but not in males.
