Megjegyzések:Purpose We measured vascular endothelial growth factor(VEGF) levels in tear fluid and serum in patients with retinalvein occlusion (RVO).Patients and methods Eight patients with RVO due to secondarymacular oedema were examined. VEGF levels were measuredby enzyme-linked immunosorbent assay. All patientshad a full ophthalmic examination (visual acuity, slit lampbiomicroscopy, perimetry, and fluorescein angiography). Centralretinal thickness (CRT) was examined using optical coherencetomography (OCT). Tear and serum samples werecollected and examinations were performed at diagnosis and1 and 4 weeks later.Results VEGF levels in the tears of RVO eyes were significantlyhigher than in fellow eyes at diagnosis and after both 1and 4 weeks (paired t test, p1=0.01, p2=0.02, p3=0.006). Wefound a weak but significant positive correlation betweenVEGF levels in tear fluid and serum of patients with RVO(r=0.21), while this correlation tended to be stronger betweenthe fellow eyes and serum levels (r=0.33).Conclusion To the best of our knowledge, we are the first toreport an increased level of VEGF in the tear fluid of patientswith RVO. Alterations of VEGF levels in tears may be usefulfor determining stages of RVO. This non-invasive and objectivemethod may also be helpful for estimating the severity ofmacular oedema and efficacy of treatment.

Megjegyzések:Retinal Vein Occlusion (RVO) is a retinal disorder resulting in a progressive, severe loss of visual acuity. Its pathogenesis has not yet been clarified. The aim of our study was to investigate cardiovascular and thrombophilic risk factors, and their interaction. 145 RVO patients and 100 control subjects were examined. Protein C, protein S, antithrombin activity, antigen levels, prothrombin G20210A gene polymorphism, factor VIII level, factor V (Leiden) mutation, plasminogen activity, lipoprotein(a), von Willebrand factor, homocysteine and fibrinogen levels and anticardiolipin antibodies were assessed. Possible relevant pathologies (e.g. diabetes mellitus, hypertension and hyperlipidemia) were also registered. Multiple logistic regression analysis revealed that hypertension was a predictive thrombophilic factor in female patients with decreased antithrombin (OR=5.69, 95% CI=2.00-16.14, p=0.0011). Male gender may be a risk factor in patients with normal blood pressure and non-elevated lipoprotein(a) level as well (OR=4.274, 95% CI=1.748-10.448, p=0.0015).Based on our results it appears that thrombophilia is likely to contribute to the development of RVO, especially if associated with cardiovascular risk factors, like hypertension. In case of justified thrombophilia mainly with hypertension, anticoagulant and/or antiplatelet therapy may be advantageous to prevent fellow eye involvement.

Megjegyzések:Background: The aim of this study was to present ophthalmological findings regarding Alport syndrome and report refractometry data and to present possible early signs of the syndrome. Material/Methods: Seven patients suffering from Alport syndrome were referred to the Department of Ophthalmology at the University of Debrecen between January 1st, 2014, and December 31st, 2015. All patients underwent slit lamp evaluation and dilated fundus biomicroscopy, with special attention paid to lenticonus and retinal changes. IOL Master, Pentacam HR, and ultrasound biomicroscopy were performed to assess keratometry, corneal thickness, anterior chamber depth, lens size, and axial length data. Results: One patient out of seven had ocular symptoms. Posterior polymorphous corneal dystrophy (PPMD) and dotand-fleck retinopathy were seen. Meanwhile, although keratoconus was not proven, remarkable astigmatism with high myopia was detected. The other six patients were found to have a significantly smaller lens diameter (an average of 7.82?0.66 mm, p=0.035) compared to normal controls (an average of 8.65?0.46 mm). Lenses also tended to be thicker in Alport patients (3.48?0.19 mm) compared to controls (3.4?0.2 mm), although the difference was not significant (p=0.394). The power of the lens also showed a significant difference (p=0.026), with Alport patients having lower lens power. Conclusions: Alport syndrome patients without classical ophthalmological findings have smaller crystalline lens diameter and lower lens power. These signs may support the diagnosis of Alport syndrome. Ophthalmologists should not only seek for the known classic signs, but also the parameters of the crystalline lens, especially if genetic testing is not available.

Megjegyzések:Secretion of cytokines and display of cytokine receptors have been reported in the orbital connective tissue in Graves' orbitopathy (GO). Lacrimal glands are putative autoimmune targets, and changes in tear film and ocular surface have also been described. Our aim was to characterize the cytokine profile of tears in Graves' patients with (GO) and without orbitopathy (GD).Methods: Tear samples were collected from 54 eyes of GO patients (age 43.4 ?15.2 years), 18 eyes of GD patients (age 46.8?11.7 years), and 24 control eyes (age 38.6?13.8 years). Patients underwent ophthalmological examination including Clinical Activity Score (CAS). The level of interleukin (IL)-1beta, IL-6, IL-13, IL-17A, IL-18, Tumor Necrosis Factor-alfa and RANTES (Regulated upon Activation, Normal T-cell Expressed, and Secreted) as well as Plasminogen activator inhibitor-1 (PAI-1) were measured by multiplex bead array and release values were calculated. Results: The release of IL-1beta, IL-6, IL-13, IL-17A, IL-18, TNF-alfa and RANTES were significantly higher in GO patients compared to controls (p<0.05). There was a 2.5-fold increase of IL-6 release. No significant differences were found in cytokine release between the GO and GD groups. In the GO group, significant positive correlation was found between CAS and the release of IL-6 and PAI-1 into tears (r=0.27, p<0.05 and r=0.24, p<0.05, respectively). PAI-1 release was significantly higher in GO than in GD patients, and was increased in both the GD and GO groups compared to controls. Conclusions: Impaired cytokine balance has been observed in tears of GO patients. Secretion of IL-6 into tears might be a useful indicator of disease activity in GO.
