Megjegyzések:Aim To look for the frequency of oesophageal dysfunctionusing radionuclide oesophageal transit scintigraphy in 145patients with undifferentiated connective tissue disease(UCTD); to seek the correlation between the clinical/laboratorydata and scintigraphic alterations; and to determinepredictive value of radionuclide oesophageal transitscintigraphy for evolution to established connective tissuedisease (CTD).Method One hundred and forty-five patients with UCTDwere examined by99mTc-DTPA oesophageal transit scintigraphy. The intraoesophageal transport of the radiopharmaceuticalwas followed and imaged by a gamma camera,a series of 128 x 128 images were stored and evaluated. The correlation between the scintigraphic data and clinicaland laboratory parameters was analysed statistically. Results Unequivocally positive scintigraphy, indicative ofmotor abnormality was found in 46% of patients (66), 7 1%(47) of whom were totally asymptomatic. Significantcorrelation was found between the presence and severityof scintigraphic alterations and antinuclear antibodies, theanti-p2GPi, IgM, IgG, the aCL antibody positivity, and theskin symptoms. Scintigraphic positivity was significantlymore frequent in patients evolving to definitive CTD

Megjegyzések:BACKGROUND:There are conflicting data on the occurrence of subclinical myocardial dysfunction in psoriatic patients and on the impact of long-term tumour necrosis factor-alpha (TNF-[alfa]) inhibitor therapy on cardiac function.OBJECTIVE:In this study, we explored whether there are any signs of subclinical cardiovascular disease (echocardiographic abnormalities) in severe psoriatic patients without clinically overt heart disease. As a second objective, the influence of long-term treatment with TNF-[alfa] inhibitors on the ventricular functions of psoriatic patients was also investigated.METHODS:Clinical and echocardiographic data from 44 psoriatic patients and 45 age- and sex-matched controls were processed. As a first step, the echocardiographic parameters of psoriatic patients obtained before anti-TNF-[alfa] treatment were compared with controls. As a second step, to detect the effect of long-term anti-TNF-[alfa] treatment on echocardiographic parameters, data of patients before and after therapy were analysed.RESULTS:The right ventricular Tei index was higher (P < 0.001), whereas the tricuspid annular plane systolic excursion (TAPSE) and right ventricular free wall peak systolic velocity were lower (P < 0.001 and P < 0.0001, respectively) in the psoriatic patients than in the controls. Following treatment with TNF-[alfa] inhibitors, TAPSE and right ventricular free wall peak systolic velocity significantly improved (P < 0.0001 for both parameters). The Tei index of both ventricles improved during biological therapy; however, this change did not reach statistical significance.CONCLUSION:Patients with severe psoriasis exhibit signs of subclinical cardiovascular disease compared to control, and prolonged anti-TNF-[alfa] therapy has a potentially beneficial effect on these signs.

Cím:Vitamin D3 levels and bone mineral density in patients with psoriasis and/or psoriatic arthritis / Gyöngyvér Kincse, Pál Harjit Bhattoa, Emese Herédi, József Varga, Andrea Szegedi, Judit Kéri, János Gaál

Megjegyzések:Limited data are available on the vitamin D3 status and bone mineral density (BMD) of patients with psoriasis orwith psoriatic arthritis. Our study intended to explore possible correlations between vitamin D status and BMD, aswell as among these parameters and the features of the underlying disorder. Seventy-two patients with psoriasis/or psoriatic arthritis (female : male ratio, 40:32; mean age, 58.5 ? 11.6 years; mean duration of follow up,142.7 ? 147.7 months) participated in the study. We evaluated the characteristic clinical features of the underlyingdisease, performed bone densitometry of the lumbar spine and the hip region, measured the serum vitamin 25(OH)D3 levels of the patients, and undertook the statistical analysis of the relationships between the clinical andthe laboratory parameters. The proportion of patients with a low BMD value did not exceed that seen in the generalpopulation. We found an inverse correlation between the serum level of vitamin 25(OH)D3 and body massindex, as well as between the former and the severity of skin involvement. Furthermore, the activity of psoriaticarthritis was significantly higher in patients with inadequate vitamin D3 status. In patients with psoriatic arthritis,BMD significantly exceeded the values measured in patients suffering from psoriatic skin lesions only. Our findingssuggest the importance of evaluating the vitamin D3 status and screening for comorbid conditions in patientswith psoriasis or psoriatic arthritis. This appears justified, in particular, due to the possible role of hypovitaminosisD3 in provoking the development of skin lesions and joint symptoms.

Cím:D3-vitamin-státusz és csontdenzitás psoriasisban és arthritis psoriaticában szenvedő betegekben / Kincse Gyöngyvér, Harjit Bhattoa Pal, Varga József, Szegedi Andrea, Kéri Judit, Gaál János
Dátum:2014
Megjegyzések:BEVEZETÉS: Kevés adat áll rendelkezésre a psoriasisban, illetve arthritis psoriaticában szenvedő betegek D3-vitamin-ellátottságával, illetve csontdenzitásával kapcsolatban. BETEGEK ÉS MÓDSZEREK: 72 psoriasisban/arthritis psoriaticában szenvedő beteg (nő/férfi arány 40/32, átlagéletkoruk 58,5 +- 11,6 év, átlagos követési idő 142,7 +- 147,7 hónap) vett részt a vizsgálatban.
