Megjegyzések:BEVEZETÉS - A rheumatoid arthritishez társuló osteoporosis hátterében számos patogenetikai faktor áll, amelyek közül egy a betegek szérumában mérhető csökkent D-vitamin-metabolit -szint. A betegek többsége szteroidot szed, és ismeretes az aktív D-vitamin-kezelés hatékonysága szteroid indukálta osteoporosisban. Emellett a D-vitaminnak számos immunszuppresszív, immunmoduláns hatása is ismert. Mindezek alapján lehet létjogosultsága az aktív D-vitamin-kezelésnek rheumatoid arthritisben. BETEGEK ÉS MÓDSZER - Huszonkilenc rheumatoid arthritises, szekunder osteoporosisban szenvedő beteget kezeltünk alfacalcidollal. A vizsgálat célja az alfacalcidol csonttömegre gyakorolt hatásának, valamint toleralhatósagának, mellékhatásainak vizsgálata volt. A hatékonyságot egyéves kezelés során, csontdenzitometriás vizsgálattal (DEXA) analizáltuk. EREDMÉNYEK - A betegek többsége a gyógyszert jól tolerálta. Hatására egy év alatt rnind a combnyak, mind a lumbalis gerinc területén mérsékelt csonttömeg-növekedés következett be. KÖVETKEZTETÉS - Az alfacalcidol, számos támadáspontja révén, alkalmas kezelési forma rheumatoid arthritishez társuló osteoporosisban. INTRODUCTION - A number of factors are involved in the pathogenesis of osteoporosis assodated with rheumatoid arthritis. One of these factors is the decreased level of active vitamin D metabolites in the sera of these patients. Many of patents with rheumatoid arthritis are treated with glucocorticoids. Active vitamin D is suitable for the treatrnent of steroid-induced osteoporosis. Fortherrnore. vitamin D exerts a number of immunosuppressive and immunomodulatory effects.Thus, vitamin D may be useful in the therapy of rheumatoid arthritis-assocated osteopenia. PATIENTS A.ND METHODS - Twenty-nine rheumatoid patients with osteoporosis were introduced into the study and treated with alfacalcidol. The aim of the study was to evaluate the tolerability and side-effects of alfacalcidol In addition, the effect of alfacalcidol on bone rnineral density was also analysed using DEXA densitometry over a one-year period. RESULTS - Most patients tolerated alfacalcidol well without significant side-effects A slight increase in bone mineral density was observed in the lumbar spine, as well as in the femoral neck after the alfacalcido! treatment. CONCLUSION - The multiple actions of alfacalcidol make this drug useful for the therapy of arthritis-associated secondary osteoporosis.

Megjegyzések:A rheumatoid arthritishez társuló szekunder osteoporosis kialakulásában számos tényező vesz részt. Az arthritis patogeneziséből eredően több olyan gyulladásos mediator termelődik a synoviumban, amely lokális csontreszorpcióhoz vezet. A betegek között sok postmenopausaíis korú nő van. Az immobilitás. a csökkent ösztrogén-,androgén- és D-vitamin-metabolit-szint, valamint a szteroid-, és esetleg a methotrexatkezelés is szerepet játszanak a csontanyagcsere zavarában. A szamos faktor miatt az arthritishez társuló csontvesztes turnovere magas. A szekunder osteoporosis terápiájában a ma ismert és használt. szinte valarnennyi gyógyszert kipróbálták már. A szerzők jelen rnunkájukban áttekintik a rheumathoid arthritishez társuló osteoporosis patogenetikai faktorait és klinikai vonatkozásait is. A number of pathogenetic íactors are involved in the secondary osteoporosis assodated to rheumatoid arthritis. Several of inflarnmatory mediators are produced in the rheumatoid synovium resulting in local bone resorption. Most of the rheumatoid patients are postmenopausal women. Apart from these factors, immobility the decteased serum levels of oestrogens, androgens and vitamin D metabolites, as weil as glucocorticoid and possibly methotrexate treatment enhance the development of osteoporosis. Rheumatoid arthritis is associated with high turnover resulting in bone resorption. A!most all antiresorptive agents have been tried in the treatment arthritis-associated osteopenia. The most important pathogenetic factors of secondary osteoporosis and the discussion or clinical aspects arc also presented.

Megjegyzések:Secondary osteoporosis is present in most patients with rheumatoid arthritis. A number of factors including inflammatory mediators, the sex, age and hormonal status of the patients, immobility, decreased levels of estrogens, androgens and vitamin D3, as well as glucocorticoid therapy play an important role in the pathogenesis of arthritis-related osteopenia. Among rheumatoid patients continuously followed at the Third Department of Medicine, more than 75 per cent have osteopenia. Most osteoporotic patients are postmenopausal women. The active vitamin D metabolite alfacalcidol is a well-tolerated antiosteoporotic agent. Its side-effects are usually mild and reversible. Authors found that alfacalcidol prevented vertebral and femoral bone loss during one-year treatment. Active vitamin D may be useful in the treatment of senile osteoporosis or in that of patients taking glucocorticoids. Calcitonin treatment also resulted in a significant bone-preserving effect. In this study the authors obtained additional data on the pathogenesis, diagnostics and treatment of osteopenia associated with rheumatoid arthritis.
