Epidural involvement is analyzed retrospectively in 512 patients with primary treatment and follow up for Hodgkin's disease (HD) between 1970 and 1999. In one case (0.2%) epidural manifestation was the first symptom and in six cases (1.2%) it occurred later, at a disseminated, advanced stage. All seven patients were male: three had mixed cellularity and four nodular sclerosis histological subtype. The thoracic segment was involved in four cases, the lumbar in two and the cervical segment in one case. The most frequent symptoms were back pain, limb weakness, paresis/plegia, incontinence. Computer tomography, magnetic resonance imaging and myelography were used as diagnostic procedures and in planning the treatment. Functional recovery was achieved by laminectomy, loco-regional irradiation and adjuvant polychemotherapy with remission of HD for 6-100 months. Later, however, six patients died due mainly to relapse/progression of HD. We emphasize the importance of an interdisciplinary approach in the treatment of HD with this relatively rare appearance, which requires close co-operation among oncohematologists, neurologists, radiologists, neurosurgeons, radiotherapists and physiotherapists.

Megjegyzések:Out of 485 patients with Hodgkin's disease (HD) treated in our institution between 1970-1998, 40 (8.2%) patients aged 65 or over at the date of the first presentation were examined retrospectively. The localization of the disease was more frequently infradiaphragmatic and rarely mediastinal in the elderly. The time that elapsed from the first clinical sign to the diagnosis of HD was twice as long as that in the young patient population. Since the first treatment was often inadequate and due to severe associated diseases, therapeutic and overall survival results in this patient population were less favourable than in the case of young patients. However, the relapse-free survival rate of the elderly patients treated successfully did not differ from that of the young patient population. Complete healing should be the final aim of treatment in the aged population, while the status of the patient, associated diseases and life expectancy not related with HD should also be taken into consideration. Besides the search for effective and less toxic treatment strategies and supportive therapy, more frequent and overall patient follow-up and care is of great importance.

Megjegyzések:A rare case of bilateral primary malignant lymphoma of breast in a 76 year old woman is presented. The lesion was examined by electron microscopy and immunochemistry. The diagnosis of primary malignant lymphoma remains a diagnosis by exclusion and requires extensive work-up to exclude widespread malignant process. The behaviour of this malignancy tends to be an aggressive one and the prognosis is generally poor.

Megjegyzések:Clinical and histopathological data of 307 patients with non-Hodgkin's lymphoma (NHL) was reviewed applying the Kiel histopathologic classification scheme and the Ann Arbor clinical and pathologic staging system. The first group included 74 patients who were diagnosed as having lympho- and reticulosarcomas during the 1965-1974 period. The case histories and histological diagnoses of these patients were retrospectively reevaluated. These patients were given orthovoltic irradiation and single agent chemotherapy. The second group included 233 patients who were entered into a prospective study from 1975 onwards. These patients were given combined polychemotherapeutical regimes. The comparative investigation has shown that the proportion of the low and high grade NHLs were the same in the groups. The administration of the new chemotherapeutical regimes did not influence significantly the survival of NHL patients.
