Megjegyzések:Background: Standard bleomycin-containing first-line therapy and/or irradiation may cause pulmonary toxicity in Hodgkin lymphoma (HL) patients. Our aim was to prospectively assess effects of chest irradiation, bleomycin administration, and other factors on lung function in the treatment of patients with HL.Methods: Pulmonary function of newly diagnosed HL patients was assessed via a St. George Respiratory Questionnaire, dynamic inhalation lung scintigraphy, spirometry, and an assessment of the diffusion capacity of the lung for carbon monoxide (DLCO) before, during, and after treatment.Results: This prospective study was conducted at the University of Debrecen. The study included 84 patients with classical HL. Most patients received standard doxorubicin, bleomycin, vinblastine, and dacarbazine chemotherapy. Both intramuscular and intravenous administrations of bleomycin were used. Brentuximab vedotin combination chemotherapy was administered to 12 patients. Mediastinal involved-field irradiation therapy (IFRT) was used to treat 16 patients. Lung scintigraphy revealed pulmonary toxicity more sensitively than DLCO. Intravenous bleomycin administration decreased diethylenetriamine pentaacetic acid clearance. Intramuscular bleomycin had the lowest level of pulmonary toxicity among considered treatments. Currently used, mediastinal IFRT had a lower level of pulmonary toxicity than bleomycin. The current prospec-tive evaluation confirmed previous results that determined that cumulative bleomycin dose and administration are major risk factors for pulmonary toxicity, while the currently used treatment method, mediastinal irradiation, was determined to be relatively safe for treating for HL patients.Conclusion: We agree with decreasing bleomycin dosage and number of cycles administered and we do not recommend avoiding mediastinal IFRT, unless multiple pulmonary risk factors are present.

Megjegyzések:AbstractIntroduction: We created a simple and effective flow cytometry scoring system (FCSS)for suspected Myelodysplastic syndromes (MDS) samples and evaluated its diagnosticand prognostic potential.Methods: Besides evaluating the four parameters suggested by Ogata, we investigatederythroid precursors and mast cells. We evaluated the six-parameterFCSS in a four-color setting (test cohort: 51 patients; 25 controls), then we implemented it into an eight-color setting and tested it on a validation cohort of patients with MDS (n=31).Results: When we compared MDS cases to non-MDS samples in the test cohort, wedetected significant differences regarding not only the four major parameters but alsotwo additional ones, namely CD71 rCV% of erythroid precursors (P=.004) and mastcell percentage (MC%) (P=.001). The utilization of the modified six-parameterFCSS provided high sensitivity and specificity both in the four color (84% and 80%, respectively) and in the eight color (81% and 100%, respectively) setting, with an excellentdiscriminative power between MDS and non-MDS samples. Furthermore, we foundsignificant difference in event-free survival between the risk groups based on themodified six-parameter FCSS (P=.001).Conclusion: We evaluated and validated a single-tube flow cytometric procedure fora simple six-parameter FCSS which has not only high diagnostic but also prognosticpower.

Megjegyzések:Introduction: Cancer survivors have an increased risk of developing further malignancies. Their development is affected by many facilitating factors, and they can occur metachronously or synchronously with the other neoplasms. Case Report: Our patient was an elderly man, who developed four primary malignancies. He had intraepithelial neoplasia of the prostate at the age of 70. Five years later, he presented with Kaposi sarcoma of skin. At 84 years of age he developed synchronous stadium IV peripheral T-cell lymphoma and adenocarcinoma of the colon. Discussion: There are different studies and theories about correlation between occurrence of lymphoid malignancies and other tumors, and it is likely that malignancies of lymphoid cells are high risk factors for developing second neoplasms.

Megjegyzések:Objective and importance: Hodgkin-lymphoma (HL) is a well-curable disease. The treatment usually includes combined multiagent conventional chemotherapy and radiotherapy. Fifth of the patients need repeated treatments because of relapse or primary progressive disease. Those HL patients, who cannot be cured at least with salvage therapy (including autologous hemopoetic stem cell transplantation [autoHSCT]), have really unfavourable prognosis. Intervention: For this heavily pretreated subset of HL patients, novel but less toxic treatment strategies should be considered. Brentuximab-vedotin is a novel targeted treatment option, which was administered after the failure of two different regimens in patients, who were ineligible for auto - HSCT or after the failure of auto - HSCT. Moreover, there are favourable data with chemotherapeutical regimens supplemented with rituximab not only in relapsed but also in newly diagnosed classical HL patients. Bendamustine, an almost forgotten 50-year-old drug lives its renessaince in the 21th century, it can be administered in refractory HL as well. Combination of the "new" and "old" drugs might be also helpful. Conclusion: Our data suggest that rituximab plus bendamustin (supplemented with or without brentuximab-vedotin) could be a suitable alternative bridging salvage therapy for heavily pretreated HL patient.

Megjegyzések:Hodgkin lymphoma shows a well-known geographic pattern, but temporal changes have been found recently as well. Patients and Methods. 439 Hodgkin lymphoma patients' clinicopathological and treatment data were processed in calendar periods of approximately ten years. The patients were treated at our department from 1980 until the end of 2008. Results. The first period (1980?89) contained 177 patients, the second (1990?99) 147, and the third (2000?08) 115 Hodgkin lymphoma patients. The mean age of the patients was 40.1, 35.9, and 36.8 years in order. The male/female ratio: 1.42, 1.45, 1.05 in order. Contrary-wise a unimodal age group pattern could have been seen with an incidence peak between 30 and 39 in the past decades. The incidence of classical mixed cellularity histological subtype is decreasing (43.7%, 58.23%, 42.6%, P=0,00098 (it is only significant in the second period)); classical nodular sclerosis shows an increasing tendency (25%, 27.32%, 34.78%,P=0.1734). The first incidence peak is predominantly created by classical nodular sclerosis, meanwhile the second peak by classical mixed cellularity. The number of early-stage patients (59.12%) is beyond the advanced stage (40%) in the last decade. Meanwhile, the number of second-stage patients was increasing (25.8%, 26.35%, 49.56%, P=0.0001) and of patients in third stage was decreasing (53.4 %, 50.67%, 20%?P<0.0001). The 5- and 10-year overall survival data were progressing: 59.7 %, 77.4 %, and 90.5 % and 44.1 %, 70.6 % and 90.5 % (expected survival) in the last decade. Conclusions. Changes can be explained by the altered nature of Hodgkin lymphoma, the changes in socioeconomic status and the development of diagnostic and therapy methods.

Megjegyzések:The characteristics of autoimmune cytopenias (AICP) associated with Hodgkin lymphoma (HL) are not thoroughly defined. We retrospectively assessed the clinical features of HL-associated AICPs in 563 HL patients diagnosed over a period of 28 years. We identified 8 cases of autoimmune hemolytic anemia (AIHA) and 8 cases of autoimmune thrombocytopenia among 14 patients altogether. Four (26%) AICPs were present at lymphoma diagnosis, while 11 (74%) cytopenias occurred during follow-up after first-line therapy. The overall incidence of HL-associated AICPs was 2.8%. Nine (75%) cytopenias responded to intravenous steroids. Seven (46%) AICPs led to the diagnosis of HL, indicated a relapse, or revealed secondary malignancies. AIHAs and AICPs altogether were more likely to develop in patients with advanced-stage HL (p = 0.010 and p < 0.004, respectively). HL patients experiencing AICPs had an increased short-term (1-year) mortality compared to the general HL population (p < 0.022). The 5-year OS of HL patients with concurrent AICPs at diagnosis was inferior compared to HL patients developing AICPs during follow-up (p = 0.005), and to HL patients without AICPs (p < 0.001). Patients with HL-associated AICPs appear to have a particular disease-related profile. The association of HL and AICPs may increase short-term mortality, while patients with concurrent AICPs at HL diagnosis have a dismal prognosis.
