Megjegyzések:Our aim was to assess whether the presence of highly active effector T-cells in atopic dermatitis (AD) is associated with changes in the number and/or function of regulatory T-cells (Tregs). Flow cytometry was utilised to determine the percentage of CD4+CD25brightCD127-/lowFOXP3+ and skin-homing CLA+CD4+CD25brightFOXP3+ Tregs in healthy controls and AD patients. The correlation between disease severity and Treg percentages was estimated. Treg suppressor activity and cell proliferation were measured after T-cell stimulation. Significantly increased percentages of Tregs were found in AD patients compared to healthy individuals, and significant correlation between the frequency of Tregs and disease severity was also detected. The otherwise normal suppressor activity of Tregs decreased in the presence of Staphylococcus enterotoxin B (SEB). In conclusion, the continuous presence of SEB can trigger an acquired functional impairment of Tregs in AD patients and the correlation between the increased frequency of Tregs and disease severity supports their important role in AD pathogenesis.

Megjegyzések:Atopic dermatitis (AD) is a chronic infl ammatory skin disease resulted partly of type I andtype IV hypersensitivity reactions triggered by T helper 2 (Th2) cells. The explanations behindthe Th2 dominance in the disease are still controversial. We addressed the question whetherthe Th2 cell domination is associated with a reduction and/or functional impairment ofCD4+CD25+FOXP3+ regulatory T cells (Treg) essential for the maintenance of Th1- Th2balance. Peripheral blood of AD patients with severe clinical symptoms and extremely highserum IgE levels (>2000 U/ml) were investigated. Flow cytometry was utilised to determinethe percentage of CD4+CD25brightFOXP3+ Tregs and CLA+CD4+CD25brightFOXP3+ Tregs in thesamples of AD patients and healthy controls. For detection of suppressor activity of CD4+CD25+Tregs, they were cocultured with CD4+CD25- effector T cells, and for T cell stimulation,anti-CD3/CD28 microbeads were applied alone or with Staphylococcus Enterotoxin B(SEB). The proliferation of the cells was measured by EZ4U colorimetric cell proliferationassay. Signifi cantly increased number of CD4+CD25brightFOXP3+ Tregs and also of CLA+CD4+CD25brightFOXP3+ Tregs were found in the peripheral blood of AD patients compared tohealthy individuals. The degree of suppressor activity of CD4+CD25+ regulatory T cells weredecreased in both anti-CD3/CD28 and SEB stimulated subgroups compared to control. Ourdata suggest coincidently with the literature that the pathogenesis of AD cannot be explainedwith the absence of CD4+CD25+FOXP3+ Treg cells, moreover the number of these cells isincreased in the peripheral blood. At the same time the function of Tregs may be impaired.

Megjegyzések:In this study we investigated whether peripheral invariant NKT cells (iNKT) can be involved in the pathogenesis of atopic dermatitis (AD). The frequency and absolute number of iNKT cells in mononuclear cells of peripheral blood of patients with AD (n=43) and healthy controls (n=15) were measured by fl ow cytometry using anti-CD3 and monoclonal antibody specifi c for the CDR3 loop of the invariant TCR a-chain of iNKT cells (clone: 6B11). The CD4/CD8 subsets of iNKT cells were also determined. Furthermore, after PMA/ionomycin stimulation for 4 hours, intracellular IFNg and IL-4 cytokines were detected in CD4+CD8-, CD4-CD8- (DN) and CD4- CD8+ subsets of iNKT cells by fi ve colour fl ow cytometry in patients with AD (n=10) and healthy controls (n=10). Both the percentage and absolute number of iNKT cells were signifi cantly lower in patients with AD (P< 0.05) compared to healthy controls. The frequency of DN and CD4-CD8+ subpopulation was signifi cantly lower in AD patient (P< 0.05). There was a positive correlation between the absolute number of DN cells and iNKT cells both in AD patients (R=938 and P< 0.001) and healthy controls (R=0.921 and P< 0.0001). The intracellular IFNg level signifi cantly decreased in DN CD4/CD8 iNKT subsets of AD patients (P<0.05), however the intracellular IL-4 level was signifi cantly increased (P< 0.05). According to these results the percentage and the number of iNKT cells and also the cytokine producing capacity of the CD4/CD8 iNKT subsets are different in peripheral blood obtained from AD patients compared to healthy controls. These observations suggest that the DN iNKT cell subset can serve as a source of IL-4 that promotes the Th2 differentiation in AD patients and might play a role in the pathogenesis of this disease.

Megjegyzések:BACKGROUND:There are conflicting data on the occurrence of subclinical myocardial dysfunction in psoriatic patients and on the impact of long-term tumour necrosis factor-alpha (TNF-[alfa]) inhibitor therapy on cardiac function.OBJECTIVE:In this study, we explored whether there are any signs of subclinical cardiovascular disease (echocardiographic abnormalities) in severe psoriatic patients without clinically overt heart disease. As a second objective, the influence of long-term treatment with TNF-[alfa] inhibitors on the ventricular functions of psoriatic patients was also investigated.METHODS:Clinical and echocardiographic data from 44 psoriatic patients and 45 age- and sex-matched controls were processed. As a first step, the echocardiographic parameters of psoriatic patients obtained before anti-TNF-[alfa] treatment were compared with controls. As a second step, to detect the effect of long-term anti-TNF-[alfa] treatment on echocardiographic parameters, data of patients before and after therapy were analysed.RESULTS:The right ventricular Tei index was higher (P < 0.001), whereas the tricuspid annular plane systolic excursion (TAPSE) and right ventricular free wall peak systolic velocity were lower (P < 0.001 and P < 0.0001, respectively) in the psoriatic patients than in the controls. Following treatment with TNF-[alfa] inhibitors, TAPSE and right ventricular free wall peak systolic velocity significantly improved (P < 0.0001 for both parameters). The Tei index of both ventricles improved during biological therapy; however, this change did not reach statistical significance.CONCLUSION:Patients with severe psoriasis exhibit signs of subclinical cardiovascular disease compared to control, and prolonged anti-TNF-[alfa] therapy has a potentially beneficial effect on these signs.

Megjegyzések:Follicular helper T (TFH) cells play crucial role in B-cell differentiation and antibody production. Although, atopic dermatitis (AD) is often associated with increased serum IgE levels, B-cell mediated responses have not been studied thoroughly. The aim of our study was to investigate the proportion of TFH-like cells in the disease. Twelve children and 17 adults with AD as well as 14 healthy controls were enrolled in the study. The frequency of CD4+CXCR5+ICOS+PD-1+ TFH-like cells and their IL-21 cytokine production were determined by flow cytometry. Immunohistochemical analysis was performed on skin biopsy specimens from AD patients for the detection of TFH markers. The percentages and absolute numbers of circulating TFH-like cells were significantly increased in children with AD compared to adult patients and healthy controls. IL-21 cytokine production of TFH-like cells was also elevated and showed a strong positive correlation with paediatric patients' SCORAD index. The expression of TFH-specific markers showed only a non-specific scattered pattern in skin biopsy specimens. This is the first study to demonstrate that TFH-like cells expanded in the peripheral blood of children with AD compared to adults. These results reinforce the importance of further investigations on TFH-like cells in different phenotypes and endotypes of AD.
