Megjegyzések:The most frequent genetic change currently known in acute myeloid leukemia (AML) is the mutation of the nucleophosmin (NPM) gene. Aberrant cytoplasmic accumulation of NPM protein (NPMc+) is the result of this mutation, and it can be demonstrated by immunohistochemistry for the identification of a favorable subgroup within "AML with normal karyotype" according to the World Health Organization classification. NPM staining pattern was defined in 71 AML and 15 control cases by the use of bone marrow smears in order to overcome limitations observed due to immunohistochemistry. In 13/71 cases (18.3%), clear cytoplasmic staining of the leukemic blast cells was detectable that was comparable with the positivity of mitotic figures physiologically lacking nuclear membrane. The biological and genetic characteristics of the NPMc+ cases determined this way were identical with the previously published results including low CD34 and HLA-DR expression and lack of recurrent karyotype abnormalities. Bone marrow smears are well applicable and therefore a real alternative for the determination of NPM with the highest accuracy for optimal risk stratification in AML.

Megjegyzések:In this study, we evaluated the suitability of the calcein assay as a routine clinical laboratory method for the identification of multidrug-resistant phenotype in acute leukaemia. This study presents the results of the calcein tests obtained in two large haematological centres in Hungary. Assays were performed with blast cells of 93 de novo acute leukaemia patients, including 65 patients with acute myeloid leukaemia (AML). Results were expressed as multidrug resistance activity factor (MAF) values. AML patients were divided into responders and non-responders and MAF values were calculated for each group. In both centres, responder patients displayed significantly lower MAF values than non-responders (P = 0.0045 and P = 0.0454). Cut-off values were established between the MAFR + SEM and MAFNR - SEM values. On the basis of these cut-off levels, multidrug resistance (MDR) negativity showed a 72% predictive value for the response to chemotherapy, whereas MDR positivity was found to have an average predictive value of 69% for therapy failure. MDR activity was a prognostic factor for survival rate and the test was suitable for detecting patients at relapse. The calcein assay can be used as a quantitative, standardized, inexpensive screening test in a routine clinical laboratory setting. The assay detects both P-glycoprotein and multidrug resistance-associated protein activities, and identifies AML patients with unfavourable therapy responses.

Megjegyzések:Six years ago (in September 2003), the fifth Haemopoietic Stem Cell Transplantation Centre of Hungary began its activity. This centre registered as the No. 648 sub-centre of the European Blood and Marrow Transplantation Committee. Objectives: To meet the demand for transplantation of haemopoietic stem cells in the North-Eastern regions of Hungary and to establish active connections with the domestic and international centres. Methods: The interventions were performed in accordance with international criteria. Results: Up to now 150 autologous stem cell transplantations were performed including 74 patients with multiple myeloma, 43 with non-Hodgkin lymphoma, one with chronic lymphoid leukaemia, 27 with Hodgkin's disease, four patients with autoimmune disease, and one with leiomyosarcoma. Survival rates were similar to Hungarian and international data. The centre has taken a role also in other activities using stem cell therapy at the University of Debrecen (dendritic cell-based vaccine program, stem cell therapy in myocardial infarction, peripheral arterial disease, and autoimmune diseases). This centre performed the most use of conditioning protocol Zevalin (ibritumomab tiuxetan), bischloroethylnitrosourea, etoposide, cytosine arabinoside, and melphalan (Z-BEAM) in patients with non-Hodgkin lymphoma in Hungary (ten patients were treated).

Megjegyzések:CD38 is expressed on the surface of leukemic cells in a significant percentage of patients with B-cell chronic lymphocytic leukaemia (CLL). From the literature it is known that CD38 expression has prognostic value in CLL, suggesting an association between CD38 expression and the mutational status of IgV genes. Periferal blood samples from 82 patients with CLL were analyzed by flow cytometry for CD38 expression on CD19+ leukemic cells. CD38 was expressed in 30 per cent or more of leukemic cells in 26 patients (patients with 30 per cent or more B cells coexpressing CD19/CD38 were considered positive). Seven of the 26 patients with high CD38 expression had advenced-stage disease (RAI III-IV). Higher than 4 mg/l levels of B2-microglobulin was measured in the serum of nine of 26 CD38+ and seven of 56 CD38-patients. Our analyses showed that the high CD38 expression is associated with other risk factors, identifying an aggresive disease, which required studies to establish the prognostic value of the high CD38 expression in early-stage disease.

Megjegyzések:In this study our purpose was to de. ne chromosomal aberrations and CD38 expression in male siblings 69 and 66-years-old with B-cell chronic lymphocytic leukemia (B-CLL). Cells from peripheral blood were analysed by comparative genomic hybridization (CGH) and fluorescence in situ hybridization ( FISH). The alteration detectable by CGH was the over-representation of the Y chromosome in both samples. Interphase FISH were performed using locus (13q14 and 17p53) and centromere ( chromosome 12, 17 and Y) specific DNA probes. One brother ( patient 1, 69 years of age) showed deletion of the 13q14 region, this alteration was associated with low CD38 expression, both predicting a favourable prognosis. However, the younger patient's ( patient 2, 66 years of age) cells expressed CD38 in high percent, which is considered as an indicator of poor prognosis, and deletion of the 13q14 was not seen. Other, relatively frequent chromosomal alterations including trisomy 12 and deletion of 17p53 were not present in any of the samples. The cytogenetic findings and the CD38 expression are in concordance with the clinico-pathological data of the siblings. Thus, we found the variability of these parameters described in B-CLL even in the familial form of the disease.
