Megjegyzések:Mannose-binding lectin (MBL) is a serum lectin synthesized by the liver and involved in innate host defense. MBL deficiency increases the risk of various infectious diseases mostly in immune-deficient conditions. Bacterial infections are a significant cause of morbidity and mortality in liver cirrhosis due to the relative immuncompromised state. Methods: Sera of 929 patients with various chronic liver diseases[autoimmune liver diseases (ALD), 406; viral hepatitis C (HCV), 185; and liver cirrhosis (LC) with various etiologies, 338] and 296 healthy controls (HC) were assayed for MBL concentration. Furthermore, a follow-up, observational study was conducted to assess MBL level as a risk factor for clinically significant bacterial infections in cirrhotic patients. Results:MBL level and the prevalence of absolute MBL deficiency (<100 ng/ml) was not significantly different between patients and controls (ALD: 14.5%, HCV: 11.9%, LC: 10.7%, HC: 15.6%). In cirrhotic patients, the risk for infection was significantly higher among MBL deficient subjects as compared to non-deficient ones (50.0% vs. 31.8%, p = 0.039). In a logistic regression analysis, MBL deficiency was an independent risk factor for infections (OR: 2.14 95% CI: 1.03?4.45, p = 0.04) after adjusting for Child?Pugh score, co-morbidities, gender, and age. In a Kaplan?Meier analysis, MBL deficiency was associated with a shorter time to develop the first infectious complication (median days: 579 vs. 944, pBreslow = 0.016, pLogRank = 0.027) and was identified as an independent predictor in a multivariate Cox-regression analysis (p = 0.003, OR: 2.33, 95% CI: 1.34?4.03). Conclusions:MBL deficiency is associated with a higher probability and shorter time of developing infections in liver cirrhosis, further supporting the impact of the MBL molecule on the host defense.

Megjegyzések:Összefoglalás Az akut, életet veszélyeztető gasztroenterológiai kórképek közül a súlyos akut pancreatitis és az akut májelégtelenség magas mortalitású kórképek, kezelésük napjainkban is kihívást jelent. Mindkét kórkép esetén jelentős sejtkárosodás jön létre, ezen felül a gyakran társuló infekció is túlzott immunválaszt, ?citokinvihart" eredményezhet, amelynek kontrollálására a gyógyszeres próbálkozások nem jelentettek kellő hatékonyságú megoldást, ezért az extracorporalis technikák kerültek előtérbe. A citokin-adszorberek közül a CytoSorb alkalmazásával van jelenleg világszerte a legtöbb tapasztalat, amely kombinálható egyéb extracorporalis szervpótló kezelésekkel is. Két esetünk kapcsán szeretnénk bemutatni a jelenlegi terápiás lehetőségeinket, illetve tapasztalatainkat. A szervezetet érintő káros hatások (pl. infekció, toxinok, trauma, autoimmun folyamatok) a sejtekből felszabaduló DAMP-ok, illetve a kórokozók által prezentált PAMP-ok révén az innate immunrendszer gyors aktivációját eredményezik, ami citokinek termelődéséhez és szisztémás gyulladás (SIRS) kialakulásához vezet. A koordinált citokinszekréció szükséges a gazdaszervezet megfelelő immunválaszának felépítéséhez és a kiváltó trigger megszűnését követően annak terminálásához. A pro-(pl. IL-1, IL-6, TNF-a) és antiinflammatorikus (pl. IL-4, IL-10) citokinek arányának felborulása, az immunválasz diszregulációja, citokinvihart eredményezhet, ami életet veszélyeztető hiperinflammációval jár. Ezáltal a kezdetben lokális gyulladás szisztémássá válhat és többszervi elégtelenséget (MODS), majd halált okozhat (1, 2).

Megjegyzések:A gastrointestinalis vérzés gyakori előfordulása, súlyossága miatt kiemelt népegészségügyi jelentőségű. Az antikoagulált és/vagy rendszeres trombocitaaggregáció-gátló készítményeket, nem-szteroid gyulladásgátlókat szedő, idősödő populáció arányának növekedésével az emésztőszervi vérzés a jövőben várhatóan számban és súlyosságban is egyre növekvő terhet ró az egészségügyi ellátó rendszerre. A gastrointestinalis vérzés ellátásában a sürgősségi szakember mellett a gasztroenterológus-endoszkóposnak van alapvető szerepe. A társszakmák (radiológus, invazív radiológus, intenzív terapeuta, sebész) képviselői sok esetben nem mellőzhetők a döntéshozatalban, a betegek ellátásában. Az érintett szakterületek képviselői szükségét érezték egy átfogó, multidiszciplináris, közösen elfogadott szemléletet tükröző gyakorlati szemléletű hazai iránymutatás összeállításának az érvényes nemzetközi irányelvek alapján. A szerzők részletesen tárgyalják a gastrointestinalis vérzés ellátásának feltételeit, a kezdeti betegészlelést, a rizikóbecslést, a laborvizsgálatokat, a betegek reszusztitációját, valamint a vérzések diagnosztikájának és terápiájának lépéseit a vérzésforrás lokalizációja szerint külön csoportosítva. A szerzők iránymutatást adnak az elsődleges ellátás sikertelensége, illetve az ismételten fellépő vérzés eseteiben, végül a sebészet mai helyét is definiálják a gastrointestinalis vérzésekben. Secondary Abstract: Gastrointestinal bleeding has outstanding impact on public health due to its high prevalence and severity. With elderly population taking more anticoagulants/antiaggregants/non-steroid anti-inflammatory drugs the digestive bleeding will certainly arise more and more challenges in quan-tity as well as in severity for the public health care system. The emergency medicine specialists and gastroenterologists have a central role in the management of patients presenting with gastro-intestinal bleeding. In certain cases radiologists, invasive radiologists, intensive care specialists and surgeons should also be involved in the decision making process and management of patients. Therefore, Hungarian experts felt the need to elaborate a comprehensive, multidisciplinary, prac-tical local guideline reflecting the frequently arisen aspects based on current international guide-lines. This guideline proposal covers topics of basic requirements, initial assessment of patients, risk evaluation, laboratory tests, hemodynamic resuscitation in case of gastrointestinal bleeding followed by its consecutive steps of diagnosis and therapy sorted by location of the source of hemorrhage. The authors give practical instructions in case of unsuccessful hemostasis or in case of rebleeding. Finally, the role of surgery is also summarized in the management of gastrointesti-nal bleeding.

Megjegyzések:OBJECTIVES: Proton pump inhibitors (PPIs) are widely prescribed to patients with liver cirrhosis. We hypothesized that long-standing PPI use is associated with spontaneous bacterial peritonitis (SBP) and accelerated development of disease-specific complications and liver-related death. METHODS: A 5-year follow-up observational cohort study assessed the impact of long-standing PPI use on the clinical course of cirrhosis in a large referral patient cohort. 350 patients with cirrhosis (males: 188, females: 162, ages: 56?6 years, alcohol: 242 [69.1%], Child-Pugh stage A/B/C: 206/108/36) were assigned to two groups: regular PPI users (n=196) and non-users (n=154). Occurrence of SBP, decompensation events (development of ascites, hepatic encephalopathy and variceal bleeding), and liver-related death were assessed. RESULTS: Regular PPI use was associated with an increased cumulative probability of SBP compared to non-users [CP: 55% vs 24.8%, HR: 4.25 (95%CI: 1.42-12.67), p=0.05], but only in patients who had no previous SBP episode (n=84). A similar association was found between regular PPI use and decompensation events. The risk of the development of a first decompensation event (ascites, HE or VB) was higher in regular PPI users compared to non-users, in patients with compensated clinical stage at enrollment (HR: 2.81, 95%CI: 1.31-6.01, p=0.008, n=146). The risk of liver-related death was also significantly increased among regular PPI users (p<0.001). In multivariate Cox-regression analysis, regular PPI use (HR: 2.81, 95%CI: 1.43-5.51, p=0.003) and MELD score (HR: 1.21, 95%CI: 1.08-1.35, p<0.001) was an independent predictor of mortality. In the present follow-up cohort study, long-term PPI use was associated with the development of SBP and a progressive disease course in patients with cirrhosis that may have been caused by enhanced pathologic BT, accelerated development of BT-dependent disease-specific complications, and liver-related death.

Megjegyzések:Recent data suggest that haplotypic variants of the DLG5 gene on 10q23 are associated with susceptibility to inflammatory bowel disease (IBD) in Germany. In view of the geographical differences in frequency of genetic markers and the absence of data in Central European patients, our aim was to determine the DLG5 R30Q variant in Hungarian IBD patients. MATERIALS AND METHODS: We investigated 773 unrelated IBD patients (age 38.1 +/- 10.3 years; duration, 8.8 +/- 7.5 years; Crohn's disease [CD]: 639; male/female, 309/330; duration, 8.4 +/- 7.1 years; ulcerative colitis [UC]: 134; male/female, 63/71; duration, 10.6 +/- 8.9 years) and 150 healthy subjects. DLG5 R30Q and TLR4 D299G variants were tested by polymerase chain reaction/restriction fragment length polymorphism. DNA was screened for NOD2/CARD15 mutations by denaturing high-performance liquid chromatography. Detailed clinical phenotype was determined by reviewing the medical charts. RESULTS: The frequency of the R30Q variant allele was not significantly different in IBD (22.0%), CD (20.8%), and UC (27.6%) patients compared with healthy control subjects (28.0%). In CD, the 113A variant allele was associated with steroid resistance (16.3% vs noncarriers, 7.6%; odds ratio [OR], 2.4; 95% CI 1.3-4.5; P = 0.013). In a logistic regression model carriage of DLG5 R30Q, perianal involvement and frequent relapses were independently associated with steroid resistance. No phenotype-genotype associations were found in UC patients, although a trend toward more extensive disease was observed in carriers of the variant allele (OR = 2.1; 95% CI 0.95-4.4; P = 0.07). CONCLUSIONS: The present data strongly contrast previous data from Germany. DLG5 113A is not associated with disease susceptibility, but there was a tendency for this allele to confer resistance to steroids. Further studies are required to evaluate the significance of DLG5 in other populations from geographically diverse regions.

Megjegyzések:In inflammatory bowel disease (IBD), enhanced inflammatory activity in the gut is thought to increase the risk of bacterial translocation and endotoxemia. In the present study we investigated the association between serum level of lipopolysaccharide-binding protein (LBP), soluble CD14 (sCD14), and clinical disease activity, high-sensitivity C-reactive protein (hs-CRP), antimicrobial serology profile, NOD2/CARD15 status, and clinical phenotype in a large cohort of Hungarian Crohn's disease (CD) patients. METHODS: In all, 214 well-characterized, unrelated, consecutive CD patients (male/female ratio: 95/119; age: 35.6 ± 13.1 years; duration:8.3 ± 7.5 years) and 110 healthy controls were investigated. Sera were assayed for LBP, sCD14, hs-CRP, ASCA IgG/IgA, anti-OMP IgA, and pANCA antibodies. NOD2/CARD15 and TLR4 variants were tested. Detailed clinical phenotypes were determined by reviewing the patients' medical charts. RESULTS: Serum LBP level was significantly higher (P < 0.0001 for both), while sCD14 was lower (P < 0.0001) in both active and inactive CD compared to the controls. The accuracy of hs-CRP (area under the curve [AUC] = 0.66), sCD14 (AUC = 0.70), and LBP (AUC = 0.58) was comparable for identifying patients with active disease. There was a significant correlation between LBP (P < 0.001), sCD14 (P = 0.015), and hs-CRP levels but not with antimicrobial seroreactivity or NOD2/CARD15 genotype. In inactive CD, LBP was associated with penetrating disease. In a Kaplan-Meier analysis and a proportional Cox-regression analysis, LBP (P = 0.006), sCD14 (P = 0.007), and previous relapse frequency (P = 0.023) were independently associated with time to clinical relapse during a 12-month follow-up period. CONCLUSIONS: Serum LBP and sCD14 are markers of disease activity in CD with a similar accuracy as hs-CRP. In addition, LBP, sCD14, and a high frequency of previous relapses were independent predictors for 1-year clinical flare-up. (Inflamm Bowel Dis 2011).

Megjegyzések:To investigate variants of immunity-related GTPase family M (IRGM) and NKX2-3 genes and genotype-phenotype in Eastern European patients with inflammatory bowel disease (IBD). METHODS: We analyzed 1707 Hungarian and Czech subjects with Crohn's disease (CD) (n = 810, age: 37.1 ± 12.6 years, duration: 10.7 ± 8.4 years) and ulcerative colitis (UC) (n = 428, age: 43.7 ± 15.0 years, duration: 12.6 ± 9.9 years), as well as 469 healthy controls. IRGM rs13361189, NKX2-3 rs10883365 and ECM1 rs13294 polymorphisms were tested by LightCycler allele discrimination. Detailed clinical phenotypes were determined by reviewing the medical charts. RESULTS: NKX2-3 rs10883365 variant allele was associated with increased risk for CD (P = 0.009, OR = 1.24, 95% CI = 1.06-1.48) and UC (P = 0.001, OR = 1.36, 95% CI = 1.13-1.63), whereas variant IRGM allele increased risk for CD (P = 0.029, OR = 1.36, 95% CI = 1.03-1.79). In contrast, ECM1 rs13294 was not associated with either CD or UC. In CD, the variant IRGM allele was associated with a colon-only location (P = 0.02, OR = 1.62, 95% CI = 1.07-2.44), whereas in UC, the ECM1 variant was associated with cutaneous manifestations (P = 0.002, OR = 3.36, 95% CI = 1.48-7.63). Variant alleles did not predict resistance to steroids or azathioprine, efficacy of infliximab, or need for surgery. CONCLUSION: NKX2-3 and IRGM are susceptibility loci for IBD in Eastern European patients. Further studies are needed to confirm the reported phenotype-genotype associations.

Megjegyzések:Infliximab (IFX) has proven to be an effective addition to the therapeutic arsenal for refractory, fistulizing, and steroid dependent Crohn's disease (CD), with efficacy in the induction and maintenance of clinical remission of CD. Our objective in this study is to report the nationwide, multicenter experience with IFX induction therapy for CD in Hungary. Methods: During a 6-year-period, beginning in 2000, a total of 363 CD patients were treated with IFX as induction therapy (5 mg/kg IFX infusions given at week 0, 2 and 6) at eleven centers in Hungary in this observational study. Data analysis included patient demographics, important disease parameters and the outcome of IFX induction therapy. Results: Three hundred and sixty three patients (183 women and 180 men) were treated with IFX since 2000. Mean age was 33.5 ? 11.2 years and the mean duration of disease was 6.7 ? 6.1 years. The population included 114 patients (31.4%) with therapy-refractory CD, 195 patients (53.7%) with fistulas, 16 patients (4.4%) with both therapy-refractory CD and fistulas, and 26 patients (7.2%) with steroid dependent CD. Overall response rate was 86.2% (313/363). A higher response rate was observed in patients with shorter disease duration (p = 0.05, OR:0.54, 95%CI:0.29-0.99) and concomitant immunosuppressant therapy (p = 0.05, OR: 2.03, 95%CI:0.165-0.596). Concomitant steroid treatment did not enhance the efficacy of IFX induction therapy. Adverse events included 34 allergic reactions (9.4%), 17 delayed type hypersensitivity (4.7%), 16 infections (4.4%), and 3 malignancies (0.8%). Conclusion: IFX was safe and effective treatment in this cohort of Hungarian CD patients. Based on our experience co-administration of immunosuppressant therapy is suggested in patients receiving IFX induction therapy. However, concomitant steroid treatment did not enhanced the efficacy of IFX induction therapy.
