Megjegyzések:Abstract Background: A lack of physical activity and overconsumption of energy dense food is associated with overweight and obesity. The neighbourhood environment may stimulate or hinder the development and/or maintenance of a healthy lifestyle. To improve research on the obesogenicity of neighbourhood environments, reliable, valid and convenient assessment methods of potential obesogenic characteristics of neighbourhood environments are needed. This study examines the reliability and validity of the SPOTLIGHT-Virtual Audit Tool (S-VAT), which uses remote sensing techniques (Street View feature in Google Earth) for desk-based assessment of environmental obesogenicity. Methods: A total of 128 street segments in four Dutch urban neighbourhoods ? heterogeneous in socio-economic status and residential density ? were assessed using the S-VAT. Environmental characteristics were categorised as walking related items, cycling related items, public transport, aesthetics, land use-mix, grocery stores, food outlets and physical activity facilities. To assess concordance of inter- and intra-observer reliability of the Street View feature in Google Earth, and validity scores with real life audits, percentage agreement and Cohen's Kappa (k) were calculated. Results: Intra-observer reliability was high and ranged from 91.7% agreement (k = 0.654) to 100% agreement (k = 1.000) with an overall agreement of 96.4% (k = 0.848). Inter-observer reliability results ranged from substantial agreement 78.6% (k = 0.440) to high agreement, 99.2% (k = 0.579), with an overall agreement of 91.5% (k = 0.595). Criterion validity was substantial to high for most of the categories ranging from 87.3% agreement (k = 0.539) to 99.9% agreement (k = 0.887) with an overall score of 95.6% agreement (k = 0.747). Conclusion: These study results suggest that the S-VAT is a highly reliable and valid remote sensing tool to assess potential obesogenic environmental characteristics.

Megjegyzések:Summary The neighbourhood is recognized as an important unit of analysis in research onthe relation between obesogenic environments and development of obesity. Oneimportant challenge is to define the limits of the residential neighbourhood, asperceived by study participants themselves, in order to improve our understandingof the interaction between contextual features and patterns of obesity. An innova-tive tool was developed in the framework of the SPOTLIGHT project to identifythe boundaries of neighbourhoods as defined by participants in five European ur-ban regions. The aims of this study were (i) to describe self-defined neighbourhood(size and overlap with predefined residential area) according to the characteristicsof the sampling administrative neighbourhoods (residential density and socioeco-nomic status) within the five study regions and (ii) to determine which individualor/and environmental factors are associated with variations in size of self-definedneighbourhoods. Self-defined neighbourhood size varies according to both individ-ual factors (age, educational level, length of residence and attachment toneighbourhood) and contextual factors. These findings have consequences forhow residential neighbourhoods are defined and operationalized and can informhow self-defined neighbourhoods may be used in research on associations betweencontextual characteristics and health outcomes such as obesity.

Megjegyzések:Abstract Background The relation between neighbourhood environmental factors and domain-specific sedentary behaviours among adults remains unclear. This study firstly aims to examine the association of perceived and objectively measured neighbourhood safety, aesthetics, destinations and functionality with transport-related, work-related and leisure-time sedentary behaviour. Secondly, the study aims to assess whether these associations are moderated by age, gender or educational level. Methods In 60 randomly sampled neighbourhoods from 5 urban regions in Europe (Ghent and suburbs, Paris and inner suburbs, Budapest and suburbs, the Randstad, and Greater London), a virtual audit with Google Street View was performed to assess environmental characteristics. A total of 5,205 adult inhabitants of these neighbourhoods reported socio-demographic characteristics, sedentary behaviours, and neighbourhood perceptions in an online survey. Generalized linear mixed models were conducted to examine associations between physical environmental neighbourhood factors and sedentary behaviours. Interaction terms were added to test the moderating role of individual-level socio-demographic variables. Results Lower levels of leisure-time sedentary behaviour (i.e. all leisure activities except television viewing and computer use) were observed among adults who perceived greater numbers of destinations such as supermarkets, recreational facilities, or restaurants in their neighbourhood, and among adults who lived in a neighbourhood with more objectively measured aesthetic features, such as trees, water areas or public parks. Lower levels of work-related sedentary behaviour were observed among adults who perceived less aesthetic features in their neighbourhood, and among adults who lived in a neighbourhood with less objectively measured destinations. Both age, gender and educational level moderated the associations between neighbourhood environmental factors and sedentary behaviours.

Megjegyzések:Background Sedentary behaviour has been associated with obesity and related chronic diseases. Disentangling the nature of this association is complicated due to interactions with other lifestyle factors, such as dietary habits, yet limited research has investigated the relation between domain-specific sedentary behaviours and dietary habits in adults. The aim of this paper was to examine the association between domain-specific sedentary behaviours and dietary habits in adults and to test the moderating effect of age and gender on this association. Methods A total of 6,037 participants from five urban regions in Europe completed an online survey, of which 6,001 were included in the analyses. Multilevel mixed-effects logistic regression analyses were used to examine main associations and interaction effects. Results All domain-specific sedentary behaviours, except transport-related sitting time, were significantly related to dietary habits. In general, having a higher sitting time was related to having less healthy dietary habits, especially for television viewing. Gender did not moderate any of the relations, and age was only a significant moderator in the relation between other leisure sitting time and alcohol consumption. Conclusion Domain-specific sitting behaviours were related to unhealthy dietary behaviours. However, the small effect sizes suggest that individual level behavioural interventions focusing on sedentary behaviour will not be sufficient to improve dietary habits. The fact that almost none of the associations were moderated by age or gender suggests that these associations, and possibly also the effects of interventions targeting both behaviours, may hold across age and gender groups.

This study aimed to examine the mediating effects of energy-balance related behaviors on the as- sociation of neighborhood socio-economic status (SES) and neighborhood residential area density (RAD) with body mass index (BMI). Methods. In total, 6037 adults from four neighborhood types (high SES/high RAD, high SES/low RAD, low SES/high RAD, and low SES/low RAD) in five Mid-European urban regions completed an online survey asking about their energy-balance related behaviors (physical activity [PA], sedentary behavior, and dietary behavior), determinants of these behaviors and their body weight and height. MacKinnon's product-of-coefficients test was used to assess mediating effects. Results. Transport-related PA, leisure-time PA and vegetable intake seemed to mediate the association between neighborhood type and BMI. Residents from low SES/low RAD neighborhoods reported less trans- port-related PA, less leisure-time PA and less vegetable intake than high SES/high RAD residents, and these behaviors (i.e. transport-related PA, leisure-time PA and vegetable intake) were related to having a higher BMI. Conclusion. The association between neighborhood type and BMI can be explained, at least in part, by energy- balance related behaviors.

This exposure assessment can then be asso-ciated with health outcomes such as obesity. Strengths of virtual audit includecollection of large amount of data, from various geographical contexts, followingstandard protocols. Using data from a virtual audit of obesity-related features car-ried out in five urban European regions, the current study aimed to (i) describe thisinternational virtual audit dataset and (ii) identify neighbourhood patterns that cansynthesize the complexity of such data and compare patterns across regions. Datawere obtained from 4,486 street segments across urban regions in Belgium, France,Hungary, the Netherlands and the UK. We used multiple factor analysis and hierar-chical clustering on principal components to build a typology of neighbourhoodsand to identify similar/dissimilar neighbourhoods, regardless of region. Fourneighbourhood clusters emerged, which differed in terms of food environment,recreational facilities and active mobility features, i.e. the three indicators derivedfrom factor analysis. Clusters were unequally distributed across urban regions.Neighbourhoods mostly characterized by a high level of outdoor recreational facil-ities were predominantly located in Greater London, whereas neighbourhoodscharacterized by high urban density and large amounts of food outlets were mostlylocated in Paris. Neighbourhoods in the Randstad conurbation, Ghent andBudapest appeared to be very similar, characterized by relatively lower residentialdensities, greener areas and a very low percentage of streets offering food andrecreational facility items. These results provide multidimensional constructs ofobesogenic characteristics that may help target at-risk neighbourhoods moreeffectively than isolated features.

Megjegyzések:Summary Socio-ecological models suggest that many elements of the social environment actas upstream determinants of obesity. This systematic review examined definitions, measures and strength of associations between the psychosocial environment and adult weight status. Studies were included if they were conducted on adults, the outcome was weight status, carried out in any developed country and investigated at least one psychosocial environmental construct. Six databases for primary studies were searched: EMBASE, MEDLINE, PsycINFO, Scopus, Web of Science and the Cochrane Library. We restricted our search to studies published in English between January 1995 and February 2015. An adapted ♭Quality Assessment Tool for Quantitative Studies' was used to evaluate risk of bias of included studies. Out of 14,784 screened records, 42 articles were assessed using full text. A total of 19 studies were included. The strongest associations with weight status were found for social capital and collective efficacy, although few studies found significant associations. There was heterogeneity in the definitions and metrics of psycho-social environmental constructs. There is limited evidence that greater social capital and collective efficacy are associated with healthier weight status. The research conducted to date has not robustly identified relations. We highlight challenges to undertaking research and establishing causality in this field and provide recommendations for further research.

Megjegyzések:ABSTRACT Objectives: To describe the design, methods and first results of a survey on obesity-related behaviours and body mass index (BMI) in adults living in neighbourhoods from five urban regions across Europe. Design: A cross-sectional observational study in the framework of an European Union-funded project on obesogenic environments (SPOTLIGHT). Setting: 60 urban neighbourhoods (12 per country) were randomly selected in large urban zones in Belgium, France, Hungary, the Netherlands and the UK, based on high or low values for median household income (socioeconomic status, SES) and residential area density. Participants: A total of 6037 adults (mean age 52 years, 56% female) participated in the online survey. Outcome measures: Self-reported physical activity, sedentary behaviours, dietary habits and BMI. Other measures included general health; barriers and motivations for a healthy lifestyle, perceived social and physical environmental characteristics; the availability of transport modes and their use to specific destinations; self-defined neighbourhood boundaries and items related to residential selection. Results: Across five countries, residents from low-SES neighbourhoods ate less fruit and vegetables, drank more sugary drinks and had a consistently higher BMI. SES differences in sedentary behaviours were observed in France, with residents from higher SES neighbourhoods reporting to sit more. Residents from low-density neighbourhoods were less physically active than those from high-density neighbourhoods; during leisure time and (most pronounced) for transport (except for Belgium). BMI differences by residential density were inconsistent across all countries. Conclusions: The SPOTLIGHT survey provides an original approach for investigating relations between environmental characteristics, obesity-related behaviours and obesity in Europe. First descriptive results indicate considerable differences in health behaviours and BMI between countries and neighbourhood types.

Megjegyzések:Abstract We examined explanatory pathways for the association between spatial access to fast food outlets and body weight in 5,076 European adults (18+). The total effect of spatial access to fast food outlets on self-reported weight status was examined using regression analyses accounting for clustering at the neighborhood level. Perceived availability and usage of fast food outlets, and fast food consumption, were considered as potential mediators and age, gender, socioeconomic status, and urban region as potential moderators. Spatial access to fast food outlets was not significantly related to weight status. Spatial access to fast food outlets was associated with perceptions about and usage of fast food outlets, and this was in turn associated with greater reported fast food consumption and unhealthier weight status. We found limited evidence for mediation effects and no evidence for effect modification.

We examined individual and contextual corre-lates of socioeconomic inequalities in neighbourhood perceptions across five urbanregions in Europe.Data were analysed from 5205 participants of the SPOTLIGHT survey.Participants reported perceptions of their neighbourhood environment with regardto aesthetics, safety, the presence of destinations and functionality of theneighbourhood, which were summed into an overall neighbourhood perceptionsscore. Multivariable multilevel regression analyses were conducted to investigatewhether the following factors were associated with socioeconomic inequalities inneighbourhood perceptions: objectively observed neighbourhood features,neighbourhood social capital, exposure to the neighbourhood, self-rated healthand lifestyle behaviours.Objectively observed traffic safety, aesthetics and the presence of destinations in theneighbourhood explained around 15% of differences in neighbourhood percep-tions between residents of high and low neighbourhoods; levels of neighbourhoodsocial cohesion explained around 52%. Exposure to the neighbourhood, self-ratedhealth and lifestyle behaviours were significant correlates of neighbourhoodperceptions but did not contribute to socioeconomic differences.This cross-European study provided evidence that socioeconomic differences inneighbourhood perceptions are not only associated with objective neighbourhoodfeatures but also with social cohesion. Levels of physical activity, sleep duration,self-rated health, happiness and neighbourhood preference were also associatedwith neighbourhood perceptions.

Megjegyzések:Background: Neighbourhood income inequality may contribute to differences in body weight. We explored whether neighbourhood social capital mediated the association of neighbourhood income inequality with individual body mass index (BMI). Methods: A total of 4126 adult participants from 48 neighbourhoods in France, Hungary, the Netherlands and the UK provided information on their levels of income, perceptions of neighbourhood social capital and BMI. Factor analysis of the 13-item social capital scale revealed two social capital constructs: social networks and social cohesion. Neighbourhood income inequality was defined as the ratio of the amount of income earned by the top 20% and the bottom 20% in a given neighbourhood. Two single mediation analyses?using multilevel linear regression analyses?with neighbourhood social networks and neighbourhood social cohesion as possible mediators?were conducted using MacKinnon's product-of-coefficients method, adjusted for age, gender, education and absolute household income. Results: Higher neighbourhood income inequality was associated with elevated levels of BMI and lower levels of neighbourhood social networks and neighbourhood social cohesion. High levels of neighbourhood social networks were associated with lower BMI. Results stratified by country demonstrate that social networks fully explained the association between income inequality and BMI in France and the Netherlands. Social cohesion was only a significant mediating variable for Dutch participants. Conclusion: The results suggest that in some European urban regions, neighbourhood social capital plays a large role in the association between neighbourhood income inequality and individual BMI.

Megjegyzések:Summary Regular cycling for transport is an important potential contributor to daily physical activity among adults. Characteristics of the physical environment are likely to influence cycling for transport. The current study investigated associations between perceived physical environmental neighbourhood factors and adults' cycling for transport across five urban regions across Europe, and whether such associations were moderated by age, gender, education and urban region. A total of 4,612adults from five European regions provided information about their transport-related cycling and their neighbourhood physical environmental perceptions in an online survey. Hurdle models adjusted for the clustering within neighbourhoods were performed to estimate associations between perceived physical environmental neighbourhood factors and odds of engaging in cycling for transport and minutes of cycling for transport per week. Inhabitants of neighbourhoods that were perceived to be polluted, having better street connectivity, having lower traffic speed levels and being less pleasant to walk or cycle in had higher levels of cycling for transport. Moderation analyses revealed only one interaction effect by gender. This study indicates that cycling for transport is associated with a number of perceived physical environmental neighbourhood factors across five urban regions across Europe. Our results indicated that the majority of the outcomes identified were valid for all subgroups of age, gender, education and across regions in the countries included in the study.
