Megjegyzések:Oral squamous cell carcinoma (OSCC) is the 7th most common malignancy and the 9th most frequent site of cancer death in Europe. Hungary occupies top places of statistics regarding OSCC incidence and mortality. Therefore, there is a high need to improve early detection. Saliva, as a readily available body fluid, became an increasingly important substance for the detection of biomarkers. Different research groups identified salivary biomarkers specific for different countries. In this study saliva samples of Hungarian OSCC patients were studiedin order to discover disease- and perhaps region-specific biomarkers. LC/MS/MS analysis on linear ion trap-Orbitrap mass spectrometer was used for qualitative and quantitative salivary protein profiling. More than 500 proteins were identified from saliva by shotgun proteomics. The up- and downregulated proteins in the saliva of patients with OSCC highlighted the importance of protein-protein interaction networks involving the immune system and proteolysis in disease development. Two potential biomarkers from our shotgun analysis and a third candidate reported earlier by a Taiwanese group were further examined by ELISA on a larger reference set of samples. Resistin reported as a biomarker in Taiwan but not validated in our study highlights the necessity of application of standardized analysis methods in different ethnic or geographical populations in order to identify biomarkers with sufficient specificity and sensitivity.

Megjegyzések:Oral squamous cell carcinoma (OSCC) accounting for about 90% of malignant oral lesionsis the 6th most common malignancy worldwide. Diagnostic delay may contribute to dismalsurvival rate therefore, there is a need for developing specific and sensitive biomarkers toimprove early detection. Hungarian population occupies the top places of statistics regardingOSCC incidence and mortality figures therefore, we aimed at finding potential salivaryprotein biomarkers suitable for the Hungarian population. In this study we investigated 14proteins which were previously reported as significantly elevated in saliva of patients withOSCC. In case of IL-1?, IL-1?, IL-6, IL-8, TNF-? and VEGF a Luminex-based multiplex kitwas utilized and the salivary concentrations were determined. In case of catalase, profilin-1,S100A9, CD59, galectin-3-bindig protein, CD44, thioredoxin and keratin-19, SRM-basedtargeted proteomic method was developed and the relative amount of the proteins wasdetermined in the saliva of patients with OSCC and controls. After several rounds of optimizationand using stable isotope-containing peptides, we developed an SRM-based methodfor rapid salivary protein detection. The validation of the selected potential biomarkers byELISA revealed salivary protein S100A9 and IL-6 as useful protein biomarkers for OSCCdetection improving the diagnostic accuracy for OSCC in the Hungarian population.A noninvasivediagnostic method to detect biomarkers useful for the early diagnosis of OSCC wasdeveloped. This can be an attractive strategy in screening saliva samples collected in anation-wide multi-centric study in order to decrease morbidity, mortality, to enhance survivalrate and to improve quality of life. The heterogeneity of protein biomarkers found in differentethnic groups presented in the literature highlights the importance of identification of population-tailored protein biomarkers.

Megjegyzések:Oral squamous cell carcinoma (OSCC) is one of the most frequent types of head and neck cancer. Despite the genetic and environmental risk factors, OSCC is also associated with microbial infections and/or dysbiosis. The secreted saliva serves as the chemical barrier of the oral cavity and, since OSCC can alter the protein composition of saliva, our aim was to analyze the effect of OSCC on the salivary chemical barrier proteins. Publicly available datasets regarding the analysis of salivary proteins from patients with OSCC and controls were collected and examined in order to identify differentially expressed chemical barrier proteins. Network analysis and gene ontology (GO) classification of the differentially expressed chemical barrier proteins were performed as well. One hundred and twenty-seven proteins showing different expression pattern between the OSCC and control groups were found. Protein?protein interaction networks of up- and down-regulated proteins were constructed and analyzed. The main hub proteins (IL-6, IL-1B, IL-8, TNF, APOA1, APOA2, APOB, APOC3, APOE, and HP) were identified and the enriched GO terms were examined. Our study highlighted the importance of the chemical barrier of saliva in the development of OSCC.

The saliva as a biological fluid for the detection of diagnostic and prognostic biomarkers has become increasingly important. Different research groups used saliva and potential salivary biomarkers specific for different countries were identified. In this study 14 proteins previously reported in the literature as significantly elevated in saliva of OSCC patients were examined. Collection of human saliva is a simple, non-invasive and cost-effective approach for screening large populations. A targeted proteomics method was developed and optimized for catalase, thioredoxin, S100A9, galectin-3 binding protein, cytokeratin 21, profilin, CD44 and CD59. Moreover the relative amounts of these proteins was determined in the saliva of OSCC patients and controls. In case of IL-1?, IL-1?, IL-6, IL-8, TNF-? and VEGF, the salivary concentrations were determined using Luminex-based multiplex immunobead assay.Based on our results, the levels of protein S100A9, thioredoxin, IL-6 and TNFa were significantly altered in OSCC patients compared to controls. The ROC analysis revealed the S100A9, thioredoxin and IL-6 as potential OSCC biomarkers applicable for the Hungarian population. In order to verify the results obtained by Luminex and SRM analyses, ELISA experiments were carried out on a multi centric study from the patients of the four University Medical Centers (Debrecen, Budapest, Pécs and Szeged) in Hungary. The salivary concentrations of IL-6, protein S100A9 and thioredoxin were determined. Based on our results the IL-6 seem to be useful biomarkers for OSCC detection in the Hungarian population.This research was supported by the Hungarian Scientific Research Fund OTKA K105034. Megjegyzések:A szájüregi laphámrák (OSCC) világszerte nagy egészségügyi problémát okozó népbetegség. A magas halálozási rátájának egyik lehetséges oka a késői diagnózis felállítása, emiatt fontos olyan biomarkerek azonosítása, mely segíthetik a betegség korai felismerését. A biomarker kutatások egyik új célpontjává váltak a fehérjékben és egyéb szerves anyagokban gazdag testfolyadékok. A nem-invazív módon gyűjthető nyálban megtalálható komponensek potenciális biomarkerként szolgálhatnak az egyes betegségek diagnosztizálásában vagy a betegség kimenetelének prognosztizálásában.Kutatásunk célja az volt, hogy olyan fehérjéket azonosítsunk, melyek biomarkerként alkalmazhatóak a magyar populációban az OSCC korai diagnózisára.Megvizsgáltuk az irodalomban fellelhető, OSCC-vel összefüggésbe hozott 14 fehérje szintjét OSCC betegektől, korban illesztett kontrolloktól, illetve fiatal kontrolloktól származó nyálmintákban SRM-alapú célzott tömegspektrometriás analízis és multiplex immunogyöngy-alapú (Luminex) módszer alkalmazásával. Emellett elvégeztük egy multicentrikus tanulmány keretei között ? a négy orvostudomány egyetemen (Debrecen, Budapest, Pécs és Szeged) ? gyűjtött nyálmintákban a potenciális biomarkerek validálását ELISA mérések segítségével.Eredményeink alapján a tioredoxin, S100A9 fehérjék, valamint az IL-6 és TNFalfa citokinek szintje szignifikáns eltérést mutatott az OSCC betegektől származó nyálmintákban a kontrollokhoz képest. A magyar populáció esetében a szignifikáns eltérést mutató négy fehérje potenciális biomarkerként alkalmazható az OSCC diagnosztikájában. A tanulmány az OTKA K105034 pályázat támogatásával készül.

Megjegyzések:Salivary IL-6 mRNA was previously identified as a promising biomarker of oral squamous cell carcinoma (OSCC). We performed a multi-center investigation covering all geographic areas of Hungary. Saliva from 95 patients with OSCC and 80 controls, all Caucasian, were collected together with demographic and clinicopathological data. Salivary IL-6 mRNA was quantified by real-time quantitative PCR. Salivary IL-6 protein concentration was measured by enzyme-linked immune-sorbent assay. IL-6 protein expression in tumor samples was investigated by immunohistochemistry. Normalized salivary IL-6 mRNA expression values were significantly higher (p < 0.001) in patients with OSCC (mean ? SE: 3.301 ? 0.885) vs. controls (mean ? SE: 0.037 ? 0.012). Differences remained significant regardless of tumor stage and grade. AUC of the ROC curve was 0.9379 (p < 0.001; 95% confidence interval: 0.8973?0.9795; sensitivity: 0.945; specificity: 0.819). Salivary IL-6 protein levels were significantly higher (p < 0.001) in patients (mean ? SE: 70.98 ? 14.06 pg/mL), than in controls (mean ? SE: 12.45 ? 3.29). Specificity and sensitivity of IL-6 protein were less favorable than that of IL-6 mRNA. Salivary IL-6 mRNA expression was significantly associated with age and dental status. IL-6 manifestation was detected in tumor cells and tumor-infiltrating leukocytes, suggesting the presence of a paracrine loop of stimulation. Salivary IL-6 mRNA is one of the best performing and clinically relevant biomarkers of OSCC.

Megjegyzések:Saliva is an easy-to access body fluid with high diagnostic potential. The utilization of saliva for oral cancer diagnosis can be an attractive possibility. Besides the oral cancer, it is important to better understand the precancerous lesions such as oral lichen planus (OLP) and leukoplakia (OLK). In order to examine the changes of salivary proteins in controls, patients with oral cancer, and patients with precancerous conditions, proximity extension assay was utilized. Some proteins and functions were characteristic to the examined groups and can serve as a starting point for further biomarker studies. The different nature of OLK and OLP was demonstrated, showing the malignant transformation and the inflammation as the prominent biological processes in the OLK and OLP, respectively. The salivary level of IL6 was verified using quantitative ELISA and the mRNA level was also studied. Elevated IL6 levels could be detected in precancerous groups compared to controls.
