Our approachcould perhaps be a practical solution to the anomalysurrounding non-standardized 25OHD values.Introduction Standardization of 25OHD values is still a challenge.We propose standardization by correction of the measured25OHD values using the linear regression equation derivedfrom the analysis of relationship between total 25OHDvalues measured by the methodology used by the laboratoryand the NIST total target values (TV) reported by the DEQASfor all 5 of the DEQAS samples in a given survey.Methods We applied our approach to standardize total25OHD values of the HunMen cohort.Results All 206 samples for the HunMen cohort were evaluatedusing the automated Liaison DiaSorin total 25OHDchemiluminescence immunoassay (CLIA). The timing ofthese measurements coincided with that of the October 2015DEQAS survey using samples 481 to 485. Following standardization,the mean total 25OHD changed from 53 to62 nmol/L and the prevalence of hypovitaminosis D(<75 nmol/L) decreased significantly from 84 to 72%.Conclusion A simple approach readily applicable at the routinediagnostic laboratory could perhaps be a practical solutionto the anomaly surrounding non-standardized 25OHD values

Megjegyzések:AbstractBackground: The aim of the present study is to evaluate serum osteoprotegerin (OPG) and soluble receptoractivator of nuclear factor ?B ligand (sRANKL) levels in a randomly selected male cohort over 50 years of age and itsassociation with cystatin C, a cysteine proteinase inhibitor that decreases formation of osteoclasts by interfering at alate stage of pre-osteoclast differentiation, apart from being a marker of renal function independent of gender,muscle mass and age; in addition to known predictors such as age, sex hormones, vitamin D, bone mineral density(BMD) and biochemical markers of bone turnover.Methods: We determined serum OPG and sRANKL levels and examined its relationship with cystatin C, age, osteocalcin,C-terminal telopeptides of type-I collagen, procollagen type 1 amino-terminal propeptide, 25-hydroxyvitamin D,parathyroid hormone, total 17?-estradiol (E2), total testosterone and L1?L4 (LS) and femur neck (FN) BMD data availablefrom194 (age, range: 51?81 years) randomly selected ambulatory men belonging to the HunMen cohort.Results: OPG correlated significantly with age (Spearman's rho (r) = 0.359, p < 0.001), cystatin C (r = 0.298, p < 0.001),E2 (r = 0.160, p = 0.028) and free testosterone index (FTI) (r = ?0.230, p = 0.001). Compared to the middle-aged(age: ? 59 years, n = 98), older men (age > 59 years, n = 96) had significantly higher serum OPG (4.6 pmol/L vs.5.4 pmol/L; p < 0.001), and lower sRANKL (0.226 pmol/L vs. 0.167 pmol/L; p = 0.048) levels. The older menshowed a significant correlation between serum OPG levels and cystatin C (Spearman's rho = 0.322, p = 0.002),and E2 (Spearman's rho = 0.211, p = 0.043). Including cystatin C and E2 in a regression model showed thatcystatin C (standard regression coefficient (?) = 0.345; p = 0.002) was the only significant predictor of serumOPG levels in the older men.Conclusions: The results of this study demonstrated that in addition to age (which was the stronger predictor),other modifiable factors such as cystatin C, FTI and E2 were also significant predictors of OPG, and that theassociation between cystatin C and OPG was more evident with increased age (older age group). As such,cystatin C is a significant predictor of OPG independently of age, FTI and E2.

Men withpsoriatic arthritis have significantly lower 25-hydroxyvitaminD levels. Men with psoriatic arthritis are at increased odds ofsuffering from hypovitaminosis D.Introduction: Skeletal manifestations as a result of abruptedbone metabolism may be predominant in psoriatic arthritis(PsA). Vitamin D plays a vital role in maintenance of skeletalhealth and is known to modulate the immune system in variousautoimmune diseases including PsA. The aim of the presentstudy was to determine the prevalence of hypovitaminosisD in a treatment naïve, de novo psoriatic arthritis male cohortin a cross-sectional, analyst blinded, age- and sex-matched,case-control study.Methods: 25 hydroxyvitamin D (25OHD), parathyroid (PTH),osteocalcin (OC) and C-terminal telopeptides of type-I collagen(CTx) levels, and lumbar spine and femoral neck bonemineral density were compared between 53 PsA and controls.Results: The prevalence of hypovitaminosis D (25hydroxyvitamin D (25OHD) levels <75 nmol/L) was 81 and57 % in the PsA and control groups, respectively. Comparedto the healthy controls, 25OHD (67.2 (12?137) nmol/L vs.51.9 (15?95) nmol/L; p=0.001) was significantly lower, andosteocalcin (13.6 (5?33) ?g/L vs. 18.2 (6?35) ?g/L; p=0.003)and C-terminal telopeptides of type-I collagen (0.20 (0.01?0.71) ?g/L vs. 0.28 (0.06?0.69) ?g/L; p=0.008) were significantlyhigher in the PsA group. A significant association wasfound between hypovitaminosis D and PsA; the odds for patientswith PsA of having hypovitaminosis D was 3.297 (95%confidence interval 1.372 to 7.922).Conclusion: The results of this study suggest that men withPsA have significantly lower 25-hydroxyvitamin D levels,and furthermore, men with PsA are at statistically significantincreased odds of suffering from hypovitaminosis D.

Megjegyzések:Célunk az országspecifikus FRAX-algoritmusáltal meghatározott 10 éves csípő- és a majorosteoporoticus törések valószínűségének, aD-vitamin-ellátottságnak, csontsűrűségnek(bone mineral density, BMD) és a csontanyagcserebiokémiai markereinek vizsgálatavolt 50 évesnél idősebb, 2-es típusú diabeteses(T2DM) férfiak csoportjában.Meghatároztuk a FRAX által kalkulált 10 évescsípő- és major osteoporoticus törések való -színűségét, a 25-hidroxi-D-vitamin-szintet[25-(OH)-D], a csontanyag csere marke reit, acsontsűrűséget az L1?L4 csigolyákon (LS) éscombnyakon (FN) összesen 68, T2DM-benszenvedő férfi esetében, és összehasonlítottukezeket az eredményeket életkorban illesztettférfiak eredményeivel (n=68).A T2DM-csoportban az átlagéletkor 61,4(51?78) év, a D-hypovitaminosis [25-(OH)-D <75 nmol/l] prevalenciája 59% volt. Azátlagos FRAX-csípőtörés és FRAX nagyosteoporoticus törés valószínűsége 0,7(0?2,8) és 3,2 (0?8,5)%-nak adódott. Az FNBMD(0,974 vs. 0,915 g/cm2, p=0,008) ésLS-BMD (1,221 vs. 1,068 g/cm2, p<0,001)szignifikánsan magasabb volt a T2DM-csoportban,mint az egészséges, életkorbanillesztett kontrollcsoportban. A 25-(OH)-Dvitamin(67,7 vs. 79,8 nmol/l, p<0,001), azI-es típusú keresztkötött kollagén C-telopeptid(?-CTx) (0,19 vs. 0,24 ?g/l, p=0,004) ésaz oszteokalcin (13,3 vs. 15,7 ?g/l,p=0,004) -szintek szignifikánsan alacsonyabbvol tak a T2DM-csoportban. A FRAXáltal meghatározott csonttörés valószín©uségei között nem volt statisztikailag szignifikánsku?lönbség a két csoport között.Mivel tudatában vagyunk, hogy vizsgálatunkesetszáma korlátozott, feltételezzu?k,hogy a T2DM-csoportban lévő magasabbBMD és a T2DM mint másodlagos kockázatitényező kihagyása a FRAX-algoritmusbólszolgálhatnak magyarázatul az irodalombanleírt és FRAX-kalkulált törésvalószínűségei közötti diszkordanciára.
