Megjegyzések:	From 1965 to 1987 authors examined 502 patients with malignant thyroid carcinoma of whom 164 developed organ or local metastases. They claim that the clinical course primarily depends on the histological subtype of the tumour and localisation of metastases. Though, the low study number does not allow to draw unequivocal conclusions it can be stated that the clinical course of highly differentiated subtype with metastases of whatever localisation is more favourable than that of the poorly differentiated tumours. Some histological varieties have better perspectives if local and not organ metastases are present. With papillary and follicular tumours the presence of brain, liver, bone, lung and lymph node metastases represent an order of improving prognosis. Anaplastic cancers and carcinosarcomas unequivocally imply bad prognosis.

Megjegyzések:	Between 1965 and 1987 the authors studied the survival of 169 patients (130 females, 39 males) suffering from follicular thyroid cancer. It is established that the factors favorably influencing the course of the disease are as follows: age below 40 years, female sex, tumor location inside the thyroid capsule. Radical surgery does not affect the survival, though it prolongs the time to the onset of metastases. In women below 40 years of age, iodine treatment performed within 1.5 months following surgery does not increase the effectiveness; therefore, its routine application is not recommended. In case of local metastases associated with hindered swallowing or respiration, external beam radiotherapy is indicated. Hormone substitution ensuring TSH restriction results in improved prognosis.

Megjegyzések:	Authors report on 250 patients with papillary thyroid cancer diagnosed and/or treated at the National Institute of Oncology, Budapest. The study population contained 195 females and 55 males (3.5:1). Patients mean age was 41,9 years. Longest survival was 293 months observed in the females under 40 years. This was followed by the survival of males under 40 years (249 months) and females over 40 years (205 months). The least favourable survival was observed with males over 40 years (137 months). Complete or nearly complete removal of the thyroid resulted in the longest survival. Survival was effected by tumour localization (intra or extracapsular position) and multifocal or nonmultifocal character. Postoperative (adjuvant) J-131 or telecobalt therapy did not affect the clinical course of the disease in patients under 40 years. Over this age limit, however, irradiation from an externatal radiation source is recommended irrespective of the patient's age or tumour localization. Hormone therapy for the reduction of TSH production improves therapeutic efficacy.
