Megjegyzések:Background: Intravenous thrombolysis (IVT) improves acute ischemic stroke (AIS) outcomes, but with limited success. In addition, ethanol potentiates the effect of r-tPA in ischemia models. Methods: The effect of acute alcohol consumption on IVT outcomes was investigated in a retrospective cohort study. AIS patients with detectable blood alcohol concentration (BAC) during IVT were included (alcohol group; n = 60). For each case, 3 control subjects who underwent IVT but denied alcohol consumption were matched in terms of age, sex, affected brain area, and stroke severity. Outcomes were determined using the NIHSS at 7 days and the modified Rankin scale (mRS) at 90 days. Results: Patients were younger and had a less severe stroke than in a standard stroke study. Favorable long-term outcomes (mRS 0?2) occurred significantly more frequently in the alcohol group compared to controls (90% vs. 63%, p < 0.001). However, the rates of hemorrhagic transformation were similar. Multiple logistic regression models identified elevated BAC as a significant protective factor against unfavorable short-term (OR: 0.091, 95% CI: 0.036?0.227, p < 0.001) and long-term outcomes (OR: 0.182, 95% CI: 0.062?0.535, p = 0.002). In patients with BAC > 0.2%, significantly lower NIHSS was observed at 3 and 7 days after IVT vs. in those with 0.01?0.2% ethanol levels. Conclusion: Elevated BAC is associated with improved outcomes in IVT-treated AIS without affecting safety.

Megjegyzések:High stroke mortality in central-eastern European countries might be due to higher stroke incidence, more severe strokes or less effective acute care than in countries with lower mortality rate. Hospital databases usually yield more detailed information on risk factors, stroke severity and short-term outcome than population-based registries. Patients and methods The Debrecen Stroke Database, data of 8088 consecutively hospitalised patients with acute cerebrovascular disease in a single stroke centre in East Hungary between October 1994 and December 2006, is analysed. Risk factors were recorded and stroke severity on admission was scored by the Mathew stroke scale. The modified Glasgow outcome scale was used to describe patient condition at discharge. Results Mean age was 68713 years, 11.4% had haemorrhagic stroke. The rate of hypertension on admission was 79% in men, and 84% in women, 40.3%of men and 19.8%of women were smokers, and 34% of all patients had a previous cerebrovascular disease in their history. Case fatality was 14.9%, and 43% had some disability at discharge. Outcome at discharge was worse with higher age, higher glucose, higher blood pressure, higher white cell count and erythrocyte sedimentation rate and more severe clinical signs on admission. In multivariate analysis admission blood pressure lost its significance in predicting outcome. Conclusions In this large Hungarian stroke unit database hypertension on admission, smoking and previous cerebrovascular disease were more frequent than in most western databases. These findings indicate major opportunities for more efficient stroke prevention in this and probably other eastern European countries.

Megjegyzések:Involvement of the brain is one of the most important complications of systemic lupus erythematosus (SLE). To investigate the correlation between abnormal cranial MRI findings and age, duration of SLE, neuropsychiatric (NP) manifestations, hypertensive status, and the presence of antiphospholipid antibodies (PA) in patients with SLE we evaluated the MRI results of 81 SLE patients in nine NP clinical subgroups.Immunoserological status was described by the presence of lupus anticoagulant (LA), and anticardiolipin antibodies (aCL). The MRI findings were categorized as normal [41], cerebral atrophy [15], small subcortical hyperintensity [7], and infarct larger than 10mm [18]. Mean age differed among the clinical subgroups (ANOVA, p = 0.002), whereas there was no age difference among the subgroups based on MRI and immunoserological results. Patients with hypertension (33/81) were a mean of 6 years older at the time of examination (p = 0.033) and had stroke more frequently, than normotensive ones (p = 0.0015). MRI abnormalities were more frequent in patients with LA positivity (p < 0.01) than in those without these antibodies, and in the hypertensive than in the normotensive subgroup (p = 0.00041). The presence of PA was associated with abnormal MRI even after controlling for the effect of age and hypertensive status (p = 0.011). In our study the MRI findings in central nervous system SLE were independent of the age of patients and the age at the diagnosis of SLE, and were not influenced by the duration of SLE; however, they were associated with immunoserological parameters and hypertension.

Megjegyzések:The central nervous system is clinically affected in about half of the systemic lupus erythematosus (SLE) patients. Mean blood flow velocities in the middle cerebral artery measured by transcranial Doppler ultrasound (TCD), before acetazolamide administration, were significantly higher in 9 patients without neurological symptoms and without MRI abnormalities (74.5 ±3.1 cm/s; mean ± SEM) than in 10 healthy controls (56 ± 2.8 cm/s) or in hemispheres of 6 SLE patients with neurological symptoms (44.5 ±2.6 cm/s). After acetazolamide, the response was significantly higher in symptom-free SLE patients (125 ± 4.6 cm/s) and lower in 6 patients with focal neurological symptoms in the affected hemisphere (66.5 ± 3.1 cm/s) compared with healthy controls (87.3 ± 4.1 cm/s). Our results suggest that the severity of impairment of the cerebral vessels is detectable by TCD in SLE patients. The acetazolamide test is suggested to be used in the follow-up of this disease.

Megjegyzések:Multiple sclerosis (MS) represents the prototypic inflammatory autoimmune disorder of the central nervous system and is the most common cause of neurological disability in young adults. These days, the management of relapsing-remitting and relapsing secondary progressive MS by immunomodulatory or immunosuppressant treatment includes 6 drugs, which have proven to be clinically and paraclinically effective. Conventional first-line disease modifying therapy currently is interferon beta and glatiramer acetate, both have a modest effect by 30% reduction on relapses. They are safe and have well tolerated side effects on long term therapy but about one third of the patients have suboptimal response to treatment. Those patient, who have failed to respond to first-line therapies are suitable for second-line treatment options including natalizumab, a humanized monoclonal antibody or immunosuppressant mitoxantron. In this review we summarize current clinical evidence on using immunomodulatory drugs for treating patients with MS.

Megjegyzések:Introduction: IV and IA thrombolysis is an effective treatment in acute ischemic stroke. Effectiveness and side effects were tested in the database. Methods: We treated 208 patients with IV/IA (145/63 pts) rt-PA after onset of ischemic stroke. After urgent CT, CTA we administered 0.9 mg/kg rt-PA according to the protocol. At local thrombolysis after 5 mg rt-PA bolus, 1 mg/min infusionwasadministered.GCS,NIHSSwereexaminedon the admission and after 7 days, mRS after 3 months. Risk factors and time window of stroke were estimated, too. Results: 64% of patients were male and 36% female.Time window was within 120min by 28% of patients, 180min 44%, 4.5h 17% and more than 270min 11% (for IA thrombolysis)afterstrokeonset.Averageagewas66.5?13.9 and 67.3?14.7 years.The most important risk factors were: hypertension (68.5%), atrial fibrillation (17%), other heart disorders(20%),smoking18.5%),diabetesmellitus(16%), hypercholesterolemia(19%),previousstrokeorTIA(16%). The average of NIHSS before thrombolysis was 14 (2-25), after 24 hours 11 (0-25). Hemorrhagic transformation was 13% after IA, 4.4% after IV administration of rt-PA. Intracerebral haemorrhage was recognized in 17.6% after IA,4.4%IV .Mortalitywas3%within24hours,21%within 3 months (IA: 27.9%, IV:13%). Large artery re-opening after IA thrombolysis was 46%. The proportion of independent patients was 36.4% (mRS) at 3 months. Conclusion: Beside IV thrombolysis IA administration of rt-PA is effective in acute stroke, but ICH and hemorrhagic transformation rate is more frequent

Megjegyzések:Objective: To assess outcomes for patients treated with interferon beta-1b immediately after clinically isolated syndrome (CIS) or after a short delay.Methods: Participants in BENEFIT (Betaferon/Betaseron in Newly Emerging MS for Initial Treatment) were randomly assigned to receive interferon beta-1b (early treatment) or placebo (delayed treatment). After conversion to clinically definite multiple sclerosis (CDMS) or 2 years, patients on placebo could switch to interferon beta-1b or another treatment. Eleven years after randomization, patients were reassessed.Results: Two hundred seventy-eight (59.4%) of the original 468 patients (71.3% of those eligible atparticipating sites) were enrolled (early: 167 [57.2%]; delayed: 111 [63.1%]). After 11 years, risk of CDMS remained lower in the early-treatment arm compared with the delayed-treatment arm (p 50.0012), with longer time to first relapse (median [Q1, Q3] days: 1,888 [540, not reached] vs 931 [253, 3,296]; p50.0005) and lower overall annualized relapse rate (0.21 vs 0.26; p50.0018). Only25 patients (5.9%, overall; early, 4.5%; delayed, 8.3%) converted to secondary progressive multiple sclerosis. Expanded Disability Status Scale scores remained low and stable, with no differencebetween treatment arms (median [Q1,Q3]: 2.0 [1.0, 3.0]). The early-treatment group had better Paced Auditory Serial Addition Task?3 total scores (p 5 0.0070). Employment rates remained high, and health resource utilization tended to be low in both groups.MRI metrics did not differ between groups.Conclusions: Although the delay in treatment was relatively short, several clinical outcomes favored earlier treatment. Along with low rates of disability and disease progression in bothgroups, this supports the value of treatment at CIS.
