Megjegyzések:High stroke mortality in central-eastern European countries might be due to higher stroke incidence, more severe strokes or less effective acute care than in countries with lower mortality rate. Hospital databases usually yield more detailed information on risk factors, stroke severity and short-term outcome than population-based registries. Patients and methods The Debrecen Stroke Database, data of 8088 consecutively hospitalised patients with acute cerebrovascular disease in a single stroke centre in East Hungary between October 1994 and December 2006, is analysed. Risk factors were recorded and stroke severity on admission was scored by the Mathew stroke scale. The modified Glasgow outcome scale was used to describe patient condition at discharge. Results Mean age was 68713 years, 11.4% had haemorrhagic stroke. The rate of hypertension on admission was 79% in men, and 84% in women, 40.3%of men and 19.8%of women were smokers, and 34% of all patients had a previous cerebrovascular disease in their history. Case fatality was 14.9%, and 43% had some disability at discharge. Outcome at discharge was worse with higher age, higher glucose, higher blood pressure, higher white cell count and erythrocyte sedimentation rate and more severe clinical signs on admission. In multivariate analysis admission blood pressure lost its significance in predicting outcome. Conclusions In this large Hungarian stroke unit database hypertension on admission, smoking and previous cerebrovascular disease were more frequent than in most western databases. These findings indicate major opportunities for more efficient stroke prevention in this and probably other eastern European countries.

Megjegyzések:Involvement of the brain is one of the most important complications of systemic lupus erythematosus (SLE). To investigate the correlation between abnormal cranial MRI findings and age, duration of SLE, neuropsychiatric (NP) manifestations, hypertensive status, and the presence of antiphospholipid antibodies (PA) in patients with SLE we evaluated the MRI results of 81 SLE patients in nine NP clinical subgroups.Immunoserological status was described by the presence of lupus anticoagulant (LA), and anticardiolipin antibodies (aCL). The MRI findings were categorized as normal [41], cerebral atrophy [15], small subcortical hyperintensity [7], and infarct larger than 10mm [18]. Mean age differed among the clinical subgroups (ANOVA, p = 0.002), whereas there was no age difference among the subgroups based on MRI and immunoserological results. Patients with hypertension (33/81) were a mean of 6 years older at the time of examination (p = 0.033) and had stroke more frequently, than normotensive ones (p = 0.0015). MRI abnormalities were more frequent in patients with LA positivity (p < 0.01) than in those without these antibodies, and in the hypertensive than in the normotensive subgroup (p = 0.00041). The presence of PA was associated with abnormal MRI even after controlling for the effect of age and hypertensive status (p = 0.011). In our study the MRI findings in central nervous system SLE were independent of the age of patients and the age at the diagnosis of SLE, and were not influenced by the duration of SLE; however, they were associated with immunoserological parameters and hypertension.

Megjegyzések:Introduction: IV and IA thrombolysis is an effective treatment in acute ischemic stroke. Effectiveness and side effects were tested in the database. Methods: We treated 208 patients with IV/IA (145/63 pts) rt-PA after onset of ischemic stroke. After urgent CT, CTA we administered 0.9 mg/kg rt-PA according to the protocol. At local thrombolysis after 5 mg rt-PA bolus, 1 mg/min infusionwasadministered.GCS,NIHSSwereexaminedon the admission and after 7 days, mRS after 3 months. Risk factors and time window of stroke were estimated, too. Results: 64% of patients were male and 36% female.Time window was within 120min by 28% of patients, 180min 44%, 4.5h 17% and more than 270min 11% (for IA thrombolysis)afterstrokeonset.Averageagewas66.5?13.9 and 67.3?14.7 years.The most important risk factors were: hypertension (68.5%), atrial fibrillation (17%), other heart disorders(20%),smoking18.5%),diabetesmellitus(16%), hypercholesterolemia(19%),previousstrokeorTIA(16%). The average of NIHSS before thrombolysis was 14 (2-25), after 24 hours 11 (0-25). Hemorrhagic transformation was 13% after IA, 4.4% after IV administration of rt-PA. Intracerebral haemorrhage was recognized in 17.6% after IA,4.4%IV .Mortalitywas3%within24hours,21%within 3 months (IA: 27.9%, IV:13%). Large artery re-opening after IA thrombolysis was 46%. The proportion of independent patients was 36.4% (mRS) at 3 months. Conclusion: Beside IV thrombolysis IA administration of rt-PA is effective in acute stroke, but ICH and hemorrhagic transformation rate is more frequent

Megjegyzések:Multiple sclerosis (MS) is primarily a disease of the central nervous system. Although the involvement of the peripheral nervous system in MS was suggested over 100 years ago, the issue is still controversial, and it is generally accepted that except for the optic nerve the peripheral nerves are left unaffected by the disease. We hypothesize, that an electroneurographical study if thorough enough, may reveal differences in some nerve conduction parameters between MS patients and healthy subjects. Second, we assume that the sensitivity of nerve conduction measurements might be increased if performed at a range of temperatures, reflecting a differential effect of cooling and warming on the peripheral nerve conduction parameters in MS patients and controls. Finally, we expect that the differences in these parameters between controls and MS patients will increase with the progression of the disease. To test these hypotheses in a pilot study, we performed a detailed analysis of the motor and sensory nerve conduction features of the right median nerve in 13 MS patients and 13 controls at 5 C increments between 20 and 40 C, and repeated these measurements after 3 years. The motor latencies were 0.3-0.6 ms longer in MS patients compared to the controls both initially and 3 years later (0.058 < p < 0.09). The durations and areas of the compound motor action potential (CMAP) appeared more sensitive to changes in temperature in the MS group (0.057 < p < 0.1). The change in both distal motor latency and sensory latency per unit change in temperature decreased significantly in 3 years within the MS but not in the control group. These results suggest a mild and progressive involvement of the PNS in MS. Most differences in this pilot study were on the border of statistical significance therefore our hypotheses should be confirmed in studies with larger sample size.

Megjegyzések:Stroke mortality is extremely high in Central-Eastern European countries. We report basic characteristics of a stroke unit in Eastern Hungary, including age and sex distribution; the proportion of transient ischemic attacks (TIA), ischemic and hemorrhagic strokes; case fatality; application of diagnostic methods; and length of stay for all patients treated with acute cerebrovascular disease over a 12-month period. Records of all patients with acute cerebrovascular disease (n = 522) discharged in 1995 from a stroke unit with a well defined catchment area of 220,000 inhabitants in Eastern Hungary were retrospectively analyzed. Case fatality was 18.6% for all patients and 21.1% after excluding cases with TIA. Computer tomography, duplex carotid ultrasound, cerebrospinal fluid examination and electroencephalography were performed in 79%, 77%, 7% and 2% of the patients, respectively. The database of the university hospital with the same catchment area was electronically searched for patients who were discharged with the diagnosis of stroke from the three departments of internal medicine. Stroke mortality data of the catchment area based on death certificates was obtained from the Central Statistical Bureau. Two hundred twenty-eight stroke deaths were reported in the catchment area in 1995. In the same period 97 stroke deaths occurred at the stroke unit and 76 at the departments of internal medicine. If we aim to treat all patients with acute stroke at the stroke unit, with the present stroke incidence and duration of hospital stay the current capacity of the stroke unit (1 bed per 10.000 inhabitants) should be doubled.

Megjegyzések:Disturbances of coagulation and fibrinolytic pathways were studied in 53 young patients with cerebral ischemia. Upon admission 26 of 53 patients had abnormality in at least one of the antithrombin-III, protein C, protein S activities or in activated protein C (APC) ratios. Three months after the first examination the majority of the previously detected abnormalities returned to normal values and the most frequent alterations were decrease in protein S activity (3 patients) and APC resistance (3 patients). Conditions resulting in impaired fibrinolysis were frequently detected upon admission. Elevation of plasminogen activator inhibitor-1, lipoprotein (a), and alpha-2-antiplasmin was present in 23, 10, and 4 cases, respectively. It is concluded that abnormalities of coagulation as well as of the fibrinolytic systems are prevalent in the acute phase of cerebral ischemia, however, the results may be significantly influenced by the disease process or the acute phase effect.
