Megjegyzések:Objective: To compare the effect of two prophylactic euvolemic fluid strategy regimens on the incidence of cerebral vasospasm and clinical outcomes in patients with aneurysmal subarachnoid hemorrhage (SAH). Methods: Ninety-six patients with a basal intravenous intake of 15 mL/kg/day of Ringer's lactate solution were included, and an additional 15 to 50 mL/kg/day Ringer's lactate (RL-group) or hydroxyethyl starch 130/0.4 solution (HES-group) was administered to maintain the targeted mean arterial pressure. The primary end point was the occurrence of cerebral vasospasm during the first 14 days. The secondary end points were case fatality, Barthel's index, and Glasgow Outcome Scores (GOS) at 30 days after SAH. Results: Cerebral vasospasm developed in 42 patients (43.7%), and nine of these events were severe. The vasospasm rate among the RL- and HES-based groups was 25/48 and 17/48, respectively. For the secondary endpoint, four patients (4%) died by the end of follow-up (two in each group). Unfavorable outcome cases were not different in the RL and HES groups (9 vs. 14, respectively). There was no difference between the Barthel's scores at 30 days between the two groups.

Megjegyzések:Background and Purpose -Spontaneous intracerebral hemorrhage is a major cause of death and disability, yet there is no convincing evidence of the benefit of any medical treatment and the role of surgery remains controversial. The international randomized Surgical Trial in Intracerebral Hemorrhage (STICH) provided an opportunity to assess the role of surgery within the centers taking part. Methods -Screening logs were completed to record details of all patients assessed by the department, whether they were included in the trial, the reasons if they were not included, and whether they underwent surgery. Results -Logs were returned by 42 centers and cover 704 months. They include details on 1578 patients with characteristics comparable to STICH inclusion criteria. Neurosurgeons were more likely to express clinical certainty about treatment for older patients, patients with a higher Glasgow Coma Score scale, and patients in whom the hematoma was located on the right or in the basal ganglia or thalamus. Patients for whom the neurosurgeon was certain about treatment were more likely to have the hematoma removed if they were younger (62 versus 68 years of age), had a lower Glasgow Coma Scale score (10 versus 13), and had a lobar hematoma (49% versus 40%). The operation rate varied between 74% in Lithuania and 2% in Hungary. Conclusions -The difference in operation rates could not be explained by differences in patient characteristics alone. This finding demonstrates the need for further evidence to ensure that treatment for intracerebral hemorrhage is not governed by local custom. ( Stroke . 2003;34:2593-2598.)

Megjegyzések:AIM: We set forth to identify predictors of symptomatic vasospasm in patients with subarachnoid hemorrhage (SAH).MATERIAL and METHODS: We used multivariate logistic regression analysis of the prospective, hospital based, single center register of the Department of Neurosurgery, University of Debrecen, Hungary. Evaluated patients' characteristics were: sex, age, Hunt-Hess grade on admission, location of aneurysm, thickness of blood clot on initial CT scan (Fisher grade), hypertension.RESULTS: Between 1987 and 2004 567 SAH cases were registered, 457 were included in this study. Symptomatic vasospasm developed in 22.5% of the cases. In univariate analysis Hunt-Hess grades 2 and 3 and female sex was predictive for symptomatic vasospasm. In multivariate analysis female sex remained significant predictor: OR: 1.8 (1.005-3.2). CONCLUSION: Women are at more danger to develop symptomatic vasospasm after subarachnoid hemorrhage.

Megjegyzések:Célkitűzés - A subarachnoidealis vérzés (SAV) statisztikai jellemzőit vizsgáltuk hazánkban 2009-ben. Módszer - Az Országos Egészségbiztosítási Pénztár felé a kórházak által a BNO-10 I60-as kódjával jelentett aktív fekvőbeteg-osztályos adatokat elemeztük. Eredmény - 2009-ben a magyarországi kórházak 1028 érintett személynél 1403 SAV-esetet jelentettek. Ez jóval magasabb a korábban közölt adatok alapján vártnál. A betegek 63,6%-a nő. Bennfekvése során az érintettek 12,2%-a halt meg. Az átlagos ápolási nap 6,47 nap volt. CT 763 személynél történt (74,2%). Hypertoniát a betegek 61,3%-ában jelentettek kísérőbetegségként. Az esetszám az életkorral nőtt az 51-60 éves korcsoportig, onnantól csökkent. A vérzésforrást 531 esetben jelölték meg. A leggyakoribb az arteria cerebri media (ACM) aneurysmája volt. Férfiakban gyakoribb az arteria communicans anterior, nőkben az ACM és az arteria carotis interna aneurysmája. A betegek 7,6%-ában fordult elő a SAV hátterében arteriovenosus malformatio. Az esetszám januárban és februárban volt a legnagyobb, áprilisban és augusztusban a legkisebb. Következtetés - A SAV gyakoribb Magyarországon, mint korábban gondoltuk.
