Megjegyzések:The potassium staining method is based on the formation of potassiumcobaltnitrite crystals after the treatment by sodium cobaltnitrite ofbrain tissue. The degree of staining correlates with the distinctpotassium content of infracted and non-infarcted brain areas.The aim of the present study was to prove that potassium stainingtechnique is a reliable method for localization of ischemic core onnative whole hemisphere cryosections of stroke patients. Furthermore,potassium stained sections have been compated with appropriatepostmortem MRI images of respective brains.Brains of stroke patients were removed within 24 h after death andpostmortem MRI scanning was performed. Horizontal cryosections offrozen brains were taken and potassium staining was performed. Usingthe stained whole hemisphere sections as "map" tissue sampling has beenmade in order to determine water and potassium content.Potassium content of infarcted samples was significantly decreased incomparison with intact regions (0.7346 +/- 0.2142 mg/L and 1.928 +/-0.447 mg/L, respectively, p<0.01) (mean values +/-SD). Water content ofaffected areas (expressed in percents) has been found to be abovenon-infarcted regions (81.657% +/- 4.07 and 72.96% +/- 6.37,respectively, p<0.01).According to our results the potassium staining method of human wholehemisphere brain sections reliably differentiates focal ischemic areasfrom intact brain regions. In conclusion, the postmortem examination ofischemic brain could be started with making the potassium map ofinfarcted whole hemisphere cryosections providing guidance for targetedtissue sampling and base of comparison for further examinations. Megjegyzések:Aims. The purpose of the present study was to evaluate predictors of outcome in primary supratentorial cerebral haemorrhage. Furthermore, we aimed to develop a prognostic model to predict 30-day fatality. Methods. We retrospectively analyzed a database of 156 patients with spontaneous supratentorial haemorrhage to explore the relationship between clinical and CT characteristics and fatal outcome within 30 days using multiple logistic regression analysis. The analyzed factors included volumetric data assessed by neuropathological and CT volumetry. A second CT scan in survivors or neuropathological ABC/2 volumetry in nonsurvivors was used along with the baseline CT to assess the growth index of haematoma. Results. Systolic blood pressure, serum potassium and glucose levels, platelet count, absolute and relative haematoma volumes, and presence and size of intraventricular haemorrhage statistically significantly predicted the fatal outcome within 30 days. Based on our results we formulated a six-factor scoring algorithm named SUSPEKT to predict outcome. Conclusions. After validation the SUSPEKT score may be applicable in general clinical practice for early patient selection to optimize individual management or for assessment of eligibility for treatment trials.
