Megjegyzések:Plasmapheresis (plasma exchange) has become a widely used therapeutic, clinical method in the last decades. During the intervention, however, the hard, mechanic surfaces of an equipment can cause essential changes in the humoral and cellular components of blood acting as danger signals to induce a state of "shear stress" or finally "complete stress" response reactions in the organism. According to earlier studies, the increase in the plasma level of adenosine is an important biochemical "danger" signal to induce "stress" responses. Its high values stimulate the production of two main "stress" hormones, adrenocorticotrop hormone (ACTH) and cortisol, which are markers for the state of "complete stress" response reaction. In the present work we measured the levels of adenosine, ACTH and cortisol, furthermore, the changes in the number of thrombocytes during plasmapheresis in blood of 12 patients with hypertrigliceridaemia. Significant elevations were found in the adenosine levels (42.54 versus 128.74 mM, p < 0.001), whereas the increases in the amounts of ACTH and cortisol were not remarkable. During a repeated plasmapheresis it was observed that the stress related part of adenosine produced during the intervention could be differentiated from that which derived from the aggregation of thrombocytes. The gradual elimination of adenosine by urine took place longer than four days. However, this situation could be regarded to be a "slight" form of stress response because of the lack of increased ACTH and cortisol levels. It was a "partial" (only adenosine linked) form of stress response, and not a "complete" one. Plasmapheresis can be regarded as a clinical intervention of "complex danger signal" if it is a "slight" one.

Megjegyzések:A szerzők összegzik az extracelluláris adenozin-trifoszfát (ATP) és adenozin (Ado) képződésének formáit, továbbá a specifikus purinerg (P2 és P1) receptoraikon keresztül kifejtett hatásaikat az egyes szervekben. Míg az ATP (P2) többnyire stimuláló, az Ado (P1) gátlási folyamatokat indít el. E reakciók nagyon szorosan egymáshoz kapcsolt rendszerét mutatja be új megvilágításban a közlemény. A szerzők bemutatják még, hogy a Pl és P2 receptorok működésének gyógyszeres befolyásolása bevált gyakorlattá vált a gyulladások, tumorok, sebészeti kórképek, a szív-, tüdő-, vese- vagy idegrendszeri betegségek gyógyításában.
