Megjegyzések:Atopic dermatitis (AD) is a chronic infl ammatory skin disease resulted partly of type I andtype IV hypersensitivity reactions triggered by T helper 2 (Th2) cells. The explanations behindthe Th2 dominance in the disease are still controversial. We addressed the question whetherthe Th2 cell domination is associated with a reduction and/or functional impairment ofCD4+CD25+FOXP3+ regulatory T cells (Treg) essential for the maintenance of Th1- Th2balance. Peripheral blood of AD patients with severe clinical symptoms and extremely highserum IgE levels (>2000 U/ml) were investigated. Flow cytometry was utilised to determinethe percentage of CD4+CD25brightFOXP3+ Tregs and CLA+CD4+CD25brightFOXP3+ Tregs in thesamples of AD patients and healthy controls. For detection of suppressor activity of CD4+CD25+Tregs, they were cocultured with CD4+CD25- effector T cells, and for T cell stimulation,anti-CD3/CD28 microbeads were applied alone or with Staphylococcus Enterotoxin B(SEB). The proliferation of the cells was measured by EZ4U colorimetric cell proliferationassay. Signifi cantly increased number of CD4+CD25brightFOXP3+ Tregs and also of CLA+CD4+CD25brightFOXP3+ Tregs were found in the peripheral blood of AD patients compared tohealthy individuals. The degree of suppressor activity of CD4+CD25+ regulatory T cells weredecreased in both anti-CD3/CD28 and SEB stimulated subgroups compared to control. Ourdata suggest coincidently with the literature that the pathogenesis of AD cannot be explainedwith the absence of CD4+CD25+FOXP3+ Treg cells, moreover the number of these cells isincreased in the peripheral blood. At the same time the function of Tregs may be impaired.

Megjegyzések:At the site of infl ammation infi ltrating CD4+ helper T cells play a pivotal role in the pathogenesis of atopic dermatitis (AD). IL-16 has been previously characterized as an immunomodulatory cytokine. Besides its chemotactic properties, IL-16 is able to trigger IL-2 receptor alpha chain expression and to activate CD4+ T cells, in synergy with IL-2 or IL-15. In addition, IL-16 amplifi es the infl ammatory processes by stimulating cytokine production in monocytes and contributes to T cell activation. Our aim was to investigate the association between serum IL-16 level and sensitisation in patients with AD. Overall 85 patients with AD were studied. Serum level of IL-16 was measured by immunoenzymatic assay in patients with AD and serum IgE, and specifi c IgE levels were also assessed. Based on specifi c IgE levels and Prick-test results, we divided AD patients into subgroups, and examined serum IL-16 and total IgE levels. Thirty-six healthy, aged-matched volunteers and 20 psoriasis patients served as controls.Compared to healthy individuals and patients with psoriasis, we found signifi cantly higher levels of IL-16 in sera of patients with AD. A signifi cant correlation was apparent between serum levels of IL-16 and the serum levels of IgE (r= 0.408; P=0.004). IL-16 levels were signifi cantly elevated in those subgroups, where specifi c IgE were detected against 1-5 or more than 5 antigens. Although the levels of IL-16 were also elevated in the Prick-test positive group, compared to Prick-test negative subjects, the difference was not signifi cant. IL-16 correlates with the degree of sensitization, but the association is not as strong as in the case of serum IgE. We assume that the role of IL-16 is complex in the pathogenesis of AD, both in the initiation and the perpetuation of the disease.

Megjegyzések:ackground Conflicting data exist on the number of invariantNKT (iNKT) cells in atopic dermatitis (AD); furthermore, nodata have been published on their functional capacity.Methods The frequency and number of circulatingCD3+6B11+ iNKT cells and their CD4+ and CD4?subpopulations were evaluated in peripheral blood obtainedfrom 41 patients with AD by four-color flow cytometry.Likewise, functional properties of iNKT cells were measuredby five-color intracellular cytokine staining.Results The number and percentage of total iNKT cells andtheir CD4/CD8 subpopulations were significantly lowerthan the controls. Of further importance, the CD4?CD8?(double negative, DN) iNKT subgroup showed thestrongest positive correlation with total iNKT cells. Inaddition, the DN subgroup exhibited the most pronouncedfunctional alteration with significantly decreasedlevels of intracellular IFN? and significantly increasedlevels of intracellular IL-4 in AD patients compared withthe controls.Conclusion The significantly altered number and cytokineproduction of iNKT cells from AD patients suggests thatthese cells may play an important role in the pathogenesisof AD.

Megjegyzések:Background: Interleukin (IL)-16 has been characterized as animmunomodulatory cytokine. Besides its chemotactic properties,IL-16 amplifies inflammatory processes and possessesimmunoregulatory functions. Our aim was to investigate theassociation between serum IL-16 levels and the degree of allergicsensitization in patients with atopic dermatitis (AD).Methods: The serum level of IL-16 was measured by immunoenzymaticassays. Eosinophil cell count, serum total andspecific IgE levels were assessed; prick tests were also carriedout. Based on specific IgE levels and prick tests, AD patientswere divided into sensitized and nonsensitized subgroups,and correlations among serum IL-16, total IgE levels and eosinophilcell counts were measured in the total patient groupand in subgroups. Results: In the total patient group, significantlyhigher levels of IL-16 were found in the sera of patientswith AD, compared to healthy individuals and patientswith psoriasis. A significant correlation was detected betweenserum levels of IL-16 and total IgE, total IgE andeosinophil counts, but not between IL-16 and eosinophils.

Megjegyzések:Regulatory T cells (Tregs) have an essential role in tolerance and immune regulation. However, few and controversial data have been published to date on the role and number of these cells in atopic dermatitis (AD). Objectives To investigate the number of CD4+CD25+FOXP3+ Tregs and interleukin 10-producing T regulatory type 1 (Tr1) cells in patients with AD. Methods Peripheral blood and skin biopsy samples from atopy patch test (APT)- positive patients with acute- and chronic-phase AD were investigated. Immunohistochemistry was applied to identify CD4+CD25+FOXP3+ Tregs in the skin, while flow cytometry was used to detect CD4+CD25highFOXP3+ Tregs and Tr1 cells in the peripheral blood. Results In the peripheral blood samples of patients with AD significantly elevated numbers of Tr1 cells were found. Although neither the absolute number nor the percentage of CD4+CD25highFOXP3+ Tregs showed significant alteration in the peripheral blood of patients, increased numbers of FOXP3+ Tregs were detected in skin biopsy specimens. All of the APT-positive skin samples showed epidermal dendritic cell aggregates, morphologically consistent with so-called Langerhans cell microgranulomas, which also contained intermingled FOXP3+ Tregs. Conclusions Tr1 cell numbers were elevated in the peripheral blood and increased numbers of CD4+CD25highFOXP3+ Tregs were detected in the skin of patients with AD. The epidermal dendritic cell clusters in APT-positive lesional skin showed a close connection to the FOXP3+ Tregs.
