Megjegyzések:A significant increase was found in the number of white blood cells, including neutrophils, in the peripheral blood of patients during plasma exchange. However, the spontaneous chemiluminescence of peripheral white blood cells (basically characterized the activated state of neutrophils) was not elevated. This finding suggests a relative deficiency in the metabolism of neutrophils during plasma exchange. This idea is supported by the observation that the levels of malonyldialdehyde in plasmas, after plasma exchange, do not increase, indicating that the activation dependent lipid peroxidation in neutrophils did not take place.

Megjegyzések:Distribution of the T-lymphocyte subpopulation capable of binding own erythrocytes was examined in patients with Hodgkin's (H) disease. Parallel with the autologous rosette-forming cells (Tar-cells) the absolute lymphocyte count and the proportion of sheep-E-rosette forming T-cells were studied in various stages and histological types of H-disease. A considerable increase in the proportion of Tar-cells was found in untreated or recurrent H-disease, whereas in the periods of remission resulting from therapy no distinct differences between these figures and the mean values of the controls were demonstrable. In the generalized stage of the disease as well as in its mixed-cell and lymphocyte depletion types of poor prognosis, the proportion of Tar-cells was increased. The absolute lymphocyte count and the proportion of T-cells showed a significant fall with the increase of Tar-cells. It is assumed on the grounds of the functional properties of T-cells that the increase in the proportion of Tar-cells in H-disease may be interpreted as a sign of suppressor hyperactivity.

Megjegyzések:The number of autologous rosette forming cells was studied in patients with SLE. The effect of steroid hormones and of sera derived from SLE patients and normal controls on autologous rosette formation by lymphocytes of SLE patients and of normal controls was examined. The number of T lymphocytes capable of recognizing autologous red cells were found to be significantly lower in the SLE patients than in the group of normal controls. Steroid hormones, whether applied in vivo or in vitro, displayed an inhibitory effect on autologous rosette formation in SLE patients and normal controls alike. The effect in vitro was dose-related, and there was no difference in sensitivity between normal and SLE lymphocytes. Pooled sera of high immunocomplex content obtained from SLE patients were also found to reduce the number of auto-rosettes. Since the lymphocytes capable of recognizing autologous red cells are known to be post-thymic precursor cells equally subject to differentiation in helper and suppression direction and play an important role in the regulatory mechanism, reduction in their number or disturbances in their function and differentiation may well provide one of the factors accounting for the impairment of immune regulation in SLE.

Megjegyzések:Therapeutical plasma exchange can be carried out by using blood cell separators based upon centrifugation or in equipments with membranes. We investigated whether activation of the complement system took place in polyvinylchloride tubes used in the blood cell separator. We also examined the changes in the classical and alternative complement pathways by an analysis of functional haemolytic titers, as well as the levels of C4, C3D, C4, albumin and alpha 1-antitrypsin. No measurable activation of the complement system was found. The decreased levels of the C3, C3d, C4, albumin and alpha 1-antitrypsin in the sera after PE could be a consequence of the haemodilution.

Megjegyzések:Functions of monocytes from the peripheral blood of 23 patients with systemic sclerosis were investigated in vitro. The yeast phagocytosis, opsonized yeast phagocytosis and binding of EA (erythrocyte-antibody) particles were found to be normal. A depressed chemotactic response was demonstrated against a zymosan-activated, complement-derived chemotactic factor. In 12 cases, monocytes were cultured for 168 hours. By the 5th and 7th days, the initially depressed chemotactic activity of monocytes returned to normal as compared to controls. This fact supports the speculation that the decreased chemotaxis cannot be caused by an intrinsic abnormality of monocytes/macrophages in systemic sclerosis.

Megjegyzések:18 patients with progressive systemic sclerosis were investigated. An absolute lymphopenia and a decrease in the number of E-rosettes and early E-rosette forming cells were found. The number of T gamma cells was reduced as compared to the control values. No diminution was found in the number of histamine receptor bearing cells but the number of T lymphocytes capable of recognizing autologous red blood cells was considerably decreased. The number and the ratio of these T cell subpopulations remained relatively stable even after 6 months in the patients with progressive systemic sclerosis. No individual correlation was found between the clinical findings and the ratio of T cell subpopulations.

Megjegyzések:Coagulation factor XIII formed by thrombin activation from zymogen factor XIII decreases the chemiluminescence (CL) of human neutrophils stimulated by opsonized zymosan (Mannozym). At high concentrations, thrombin and plasmin also decreased the CL induced by opsonized zymosan. The inhibitory effect of all the three enzymes was due to their influence on the cell membrane receptors (C3b and Fe) and not to their direct effect on opsonized Mannozym. The potential clinical role of factor XIII, thrombin and plasma in the regulation of neutrophil functions is assumed.

Megjegyzések:One of the first steps in the T cell activation is modulation of T3-Ti complex from the cell membrane. This process was studied in patients with SLE by immunofluorescence staining and measured in a cell sorter. The modulation of T3 antigen was impaired in a number of patients. The mitogen-induced proliferation was also decreased in these cases, but decreased blast transformation occurred also with efficient modulation. Pretreatment of normal T lymphocyte with sera from patients with decreased modulation caused only moderate inhibition of T3 antigen modulation. Modulation in vitro was not influenced by steroids.
