Megjegyzések:Objectives: This study aimed to investigate the correlation between cervicovaginal fluid zinc levels and participants' menopausal status and the presence of vulvovaginal symptoms of genitourinary syndrome of menopause. Methods: Women included in the study underwent a pelvic examination, collection of vaginal cytology, and cervicovaginal lavage was performed in a standardized fashion. The cervicovaginal lavage zinc content was determined. The vaginal health index was calculated, and the visual analog scale was obtained based on the participant's burning, itching, vaginal pain, dyspareunia, dryness, and dysuria. Results: One hundred twenty women participated in the study. Eighty-two participants were menopausal. The mean vaginal health index was 15 ? 6, while the total visual analog scale score was 14 ? 14. We found significant differences between cervicovaginal lavage zinc level and age group, menopausal status, presence of vaginal dryness, and vaginal atrophy. We also found a moderate positive correlation between Vaginal Maturation Value and cervicovaginal lavage zinc levels (r = 0.495, P value < 0.01) in the correlation analysis. However, in the multivariate regression model, including all significant factors, only vaginal atrophy remained significant. Conclusions: We found a significant association between vaginal atrophy (vaginal maturation value < 50) and cervicovaginal lavage zinc levels. Zinc levels of cervicovaginal lavage could be used as a new marker of vaginal atrophy.

Megjegyzések:Background: Stress urinary incontinence (SUI) is a common condition that requires proper evaluation to select a personalized therapy. Vaginal Tactile Imaging (VTI) is a novel method to assess the biomechanical parameters of the pelvic floor. Methods: Women with SUI were enrolled in this cross-sectional study. Participants completed the Medical, Epidemiologic, and Social Aspects of Aging (MESA) questionnaire and the Patient Global Impression of Severity Question (PGI-S) and underwent a VTI examination. Based on the MESA and PGI-S questionnaires, participants were divided into mild, moderate, and severe SUI groups. Fifty-two biomechanical parameters of the pelvic floor were measured by VTI and compared between the groups (mild vs. moderate and severe). SUI Score and Index were calculated from the MESA questionnaire. Pearson correlation was used to determine the strength of association between selected VTI parameters and the MESA SUI Index and MESA SUI Score. Results: Thirty-one women were enrolled into the study. Significant differences were observed in the VTI parameters 16, 22-24, 38, 39 when the difference between mild and severe subgroups of SUI based on the PGI-S score was examined. Parameter 16 refers to the maximum gradient at the perineal body, parameter 22-24 refers to the pressure response of the tissues behind the vaginal walls, and parameter 38, 39 refers the maximum pressure change and value on the right side at voluntary muscle contraction. VTI parameter 49, describing the displacement of the maximum pressure peak in the anterior compartment, showed a significant difference between the mild SUI and the moderate-severe SUI according to the MESA SUI score (mean ? SD 14.06?5.16 vs. 7.54?7.46, P=0.04). The MESA SUI Index and SUI Score displayed a positive correlation concerning VTI parameters 4 (the maximum value of the posterior gradient) and 27 (the displacement of the maximum pressure peak in the anterior compartment) (VTI4 vs. MESA SUI Index r=0.373,P=0.039; VTI4 vs. MESA SUI Score r=0.376,P=0.037; VTI27 vs. MESA SUI Index r=0.366,P=0.043; VTI27 vs. MESA SUI Score r=0.363,P=0.044). Conclusions: Female pelvic floor biomechanical parameters, as measured by VTI, correlate significantly with the severity of SUI and may help guide therapeutic decisions.

Megjegyzések:Introduction: Zinc deficiency is closely linked to impaired mucosal integrity which is critical for the prevention of infections. The presence of zinc in the epidermis promotes epidermal homeostasis. Oral supplementation with zinc had no significant impact on cervicovaginal lavage zinc level despite a significant rise in serum zinc level. However, vaginal application of zinc in the form of a zinc-containing vaginal gel is undoubtedly a viable option to deliver zinc to the vagina. Zinc transporters are present in the vagina and ZIP4 is present in the vagina wall for absorption of vaginal zinc replenishment. Objective: To test the hypothesis that prophylactic use of zinc containing vaginal gel decreases the reoccurrence rate of vaginal infection in women with a diagnosis of recurrent vaginal infections. Methods: Women with a history of recurrent vaginal infections (bacterial vaginosis or recurrent vulvovaginal candidiasis) were offered prophylactic treatment with a commercially available zinc-containing vaginal moisturizer gel. Women were asked to use the vaginal gel daily for two weeks and after that twice per week. Women were asked to return to the clinic if any symptoms of vaginal infection were present for evaluation. Results: Eight women were enrolled. The mean age was 32 ? 6 years, mean BMI 24 ? 5, Gravida 1.5 (0-3), Para 1.5 (0-3). All women were premenopausal. None of the patients were using any oral or vaginal hormonal treatment. The number of sexual partners was 1 (1-3). On average, there was at least one infection every three months before treatment. After treatment with the zinc-containing vaginal gel, 5 out 8 women did not have an infection in the first three months (P=0.04). Three women developed one vaginal infection resulting in a 62% reduction in infections. Conclusions: Zinc-containing vaginal gel may help to prevent recurrent vaginal infections. Further studies are required to delineate the possible mechanism of action: zinc - immunoprotective -immunomodulation through vaginal dendritic cells / Langerhans cells; lactic acid ? acidic pH favors lactobacilli and prevents overgrowth of other organisms; hydroxyethyl cellulose - gentle exfoliation - increased availability of glycogen, favors lactobacilli growth.

Megjegyzések:Introduction: Vaginal laser treatment became a popular treatment modality for genitourinary syndrome of menopause. However scientific data is limited regarding the effects of laser treatment on different pelvic floor dysfunctions. Objectives: To evaluate the effect of the Mona Lisa Touch CO2 laser treatment on postmenopausal women vulvovaginal and pelvic floor dysfunction symptoms. Methods: Forty-three postmenopausal women were enrolled and underwent vaginal laser treatment with. Mona Lisa Touch. Patients received three vaginal laser treatments with the 360 degree probe 4-6 weeks apart. Pelvic Organ Prolapse Distress Inventory 6 (POPDI-6), Colorectal?Anal Distress Inventory 8 (CRADI-8) and Urinary Distress Inventory 6 (UDI-6) validated questionnaires were filled out by each patient before each session and 4 weeks after the final treatment. Vaginal Health Index (Elasticity, Fluid Secretion, pH, Epithelial mucosa, Moisture) was calculated before each session and 4 weeks after the final treatment. In addition, patients were asked to fill out a Visual Analog Scale (1-10, lower scores indicates less bother) on vaginal pain, dryness, burning, itching, dyspareunia and dysuria. Paired ttest was used to compare the before and after treatment results. Results: All patients were in menopause. The average age was 57?10 years. POPD-6 standardized scores were not significantly different after the first treatment (20 ?19 vs. 14?11 after the first treatment). But after the second treatment there was a significant improvement in the standardized score to 11?12 ( P=0.02). After the third treatment the score was even lower 10?13 (P=0.01). CRADI-8 standardized scores did not change significantly after three laser treatment (16 ?18 vs. 13?17 after the 1st treatment/13?16 after the 2nd treatment/ 12?18 after the 3rd treatment). UDI-6 standardized scores were not significantly different after the first laser treatment (32?23 vs. 26?23 after the first treatment). But after the second treatment there was a significant improvement in the standardized score to 25 ? 20 ( P=0.03). The Vaginal Health Index was 15?5 before and 17?4 after the first treatment, 19?5 after the second and 21?4 after the third treatment. VHI was significantly improved after each treatment (P<0.01). The largest incremental change occurred in vaginal moisture, followed by vaginal fluid and elasticity. The patient reported combined Visual Analog Score (VAS) was 16?16 before and 9?12 after the first treatment, 6 ? 9 after the second and 3 ? 8 after the third treatment. VAS was significantly lower (P<0.01) after each laser treatment. The largest improvement occurred after the first laser treatment, but each additional treatment added additional symptoms relief. Conclusion: CO2 vaginal laser treatment significantly improves patient reported urinary and pelvic organ prolapse bother. The minimum of two laser treatments were needed to achieve a significant benefit. In addition, CO2 vaginal laser treatment significantly improves both the patient reported subjective symptoms and the Vaginal Health Index in postmenopausal women even after a single laser treatment.

Megjegyzések:Objectives: While vaginal wall elasticity is an important aspect of maintaining good vaginal health, the factors that determine vaginal elasticity in the postpartum period are relatively poorly understood. Our study aimed to evaluate the contribution of the levator muscle injury to vaginal wall elasticity in postpartum women. Methods: A cross-sectional study was performed six weeks after vaginal delivery. Elastographic parameters of the vaginal walls were measured using strain ultrasound elastography in four regions of interest (ROI) in the anterior and four in the posterior vaginal wall. The Elastography Index (EI) was defined as the average colour score of four ROIs on each side of the vagina. The levator-urethra gap (LUG) was measured on 3D ultrasound images as the distance between the centre of the urethra and the levator insertion bilaterally. Based on previous studies, an abnormal LUG?25mm indicated levator avulsion. Groups were compared with unpaired t-tests. Results: Fifty-one subjects were included in the study (14 had unilateral, 11 had bilateral avulsion, and 27 had no sign of avulsion). The EI did not differ significantly between any defect (unilateral and bilateral defect) and no-defect groups measured on the anterior vaginal wall (mean?SD, 25.03?23.65 vs. 25.30?22.73, NS) and also on the posterior vaginal wall (mean?SD, 38.68?24.67 vs. 46.31?16.48, NS). Similarly, we examined EI between the bilateral avulsion and no-defect groups, where there was also no significant difference in either the anterior (mean?SD, 24.1?0.83 vs. 25.30?22.73, NS) or posterior (mean?SD, 46.02?14.71 vs. 46.31?16.48, NS) vaginal wall. Conclusions: Our results suggest that levator muscle injury may not contribute significantly to vaginal elasticity in postpartum women.

Megjegyzések:Objective: Our study aimed to improve the understanding of the biomechanical changes occurring in the pelvic floor due to pelvic floor muscle training (PFMT), which is responsible for improving stress urinary incontinence (SUI) symptoms. Study design: In this prospective cohort study, we examined the impact of a six-week PFMT program in women with stress or stress-predominant urinary incontinence on the biomechanical parameters of the pelvic floor. Fifty-two biomechanical parameters were measured by Vaginal Tactile Imager (VTI) at baseline and at a six-week follow-up. In addition, we have assessed the Urinary Distress Inventory (UDI-6), Incontinence Impact Questionnaire (IIQ-7), and Patient's Global Impression of Severity (PGI-S) scores at enrolment and at six-week follow-up. We have divided our cohort into two groups, Improved Group (IG) and Not-improved Group (NIG), based on the change in UDI-6 score after six weeks and compared the biomechanical changes within and between the two groups. Results: The overall cohort included twenty women with stress or stress-predominant SUI based on MESA questionnaire [MESA SUI index (mean ? SD) 63.3 % ? 24.0 %, MESA UUI index (mean ? SD) 13.3 % ? 15.3 %, p < 0.01]. Nine of the 52 VTI parameters have changed significantly after six weeks of PFMT in the entire cohort. Eleven women (IG) achieved the minimum 11-point change in the UDI-6 scaled score. In IG, we found that three VTI parameters differed significantly: Parameter 1: the maximum value of force measured during the VTI probe insertion [(N, mean ? SD) 0.55 ? 0.18 vs. 0.78 ? 0.31, p < 0.049], parameter 18: the maximum gradient at the upper third of the vagina (posterior) [(kPa/mm, mean ? SD) 0.16 ? 0.05 vs. 0.36 ? 0.28, p < 0.048], and parameter 47: integral force change in the anterior compartment at reflex pelvic muscle contraction (cough) [(N, mean ? SD) 1.61 ? 0.85 vs. 1.97 ? 0.71, p < 0.045]. Conclusion: Our study revealed a significant association between the improvement of strength in targeted muscle groups and a reduction in urinary incontinence symptoms. Identifying specific muscles changing PFMT provides valuable insights for specific interventions. Our findings may help to create personalized and targeted interventions to improve the quality of life of women affected by SUI.

Megjegyzések:Objectives: Strain elastography is a novel method for assessing tissue elasticity. We aimed to evaluate the value of vaginal strain elastography in women with anterior and/or posterior vaginal compartment prolapse six weeks postpartum. Methods: Women with or without anterior or posterior vaginal prolapse were included in this cross-sectional study at six weeks after vaginal delivery. Participants underwent a detailed vaginal examination with an assessment of POP-Q. Prolapse was defined as Ba and/or Bp?-1. Elastography parameters of the vaginalwalls were measured in nine regions of interest (ROI). The Elastography Index (EI) was defined by the average colour score of nine ROIs. Groups were compared with unpaired t-test or Mann-Whitney U test. Results: 24 women were diagnosed with anterior and/or posterior vaginal wall prolapse, and 27 had no signs of prolapse six weeks postpartum. The two groups had no significant differences in age (years, mean?SD, 29?3 vs. 28?3, NS), BMI (mean?SD, 26.8?3.7 vs. 28.6?4.0, NS), and birthweight (Gramm, mean?SD, 3273?417 vs. 422?400, NS). EI was significantly lower in the prolapse group than in the non-prolapse group (mean?SD, 27?17 vs. 38?18, p<0.05). Conclusions: Vaginal elasticity is significantly decreased in women with vaginal prolapse, measured by elastography index six weeks postpartum. Our results suggest that strain elastography may be a valuable tool in the assessment of vaginal prolapse postpartum and pave the way for further studies investigating the role of aginal wall elastography in predicting postpartum prolapse.

Megjegyzések:Background: The basic principle of vaginal laser therapy is the rejuvenation of the affected tissue. Zinc and copper are essential nutritional trace elements and have a key role in connective tissue homeostasis. We aimed to investigate the effect of vaginal, fractional CO2 laser treatment on cervicovaginal lavage (CVL) zinc and copper levels. Methods: Twenty-nine postmenopausal women with symptoms of vaginal dryness were enrolled in our prospec-tive cohort study. Three treatments with MonaLisa Touch CO2 laser system were performed four weeks apart. At each treatment CVL was collected, Vaginal Health Index (VHI) was obtained, and Visual Analog Scale (VAS) for vaginal dry-ness was assigned by patients. Zinc and copper concentrations were measured with optical emission spectrometry before each treatment and six weeks after the 3rd treatment. Results: The VHI scores significantly improved after each laser treatment (mean ?SD VHI score, 13.03 ? 4.49 before vs. 15.55 ?4.35 after the 1st, 17.79 ?4.57 after the 2nd and 19.38 ?4.39 after the 3rd treatment, P < 0.01). Similarly, VAS scores reflected improvement (mean ?SD VAS score 6.59 ?2.86 before vs. 4.17 ?2.86 after the 1st, 2.45 ? 2.43 after the 2nd and 1.41 ?1.94 after the 3rd treatment, P < 0.01). CVL zinc levels were significantly higher compared to copper levels (0.06 ?0.04 vs. 0.006 ?0.006 mg/L, P < 0.01) at baseline. While copper levels remained the same through treatments, the CVL zinc level was significantly higher after the second laser treatment compared to the baseline. Conclusions: Fractional CO2 laser treatment of the vagina impacts CVL zinc and copper levels differently. While CVL copper levels were not different after each laser treatment, zinc levels were significantly higher after the second treat-ment before returning to baseline values.
